A temporary procedure change has been issued to ensure the valve is open during future discharges.

From a technical viewpoint," the temporary monitor was satisfactory.

However, the modification to the system was not properly administratively controlled.

The existing procedures made no mention of the valve line-up necessary to provide a sample flow for the temporary monitor.The personnel involved have been made aware of their error.The valve supplying sample flow to the temporary monitor.has been tagged open.A temporary procedure change has been issued for controlled liquid releases.The temporary;system will be removed as soon as repairs are complete.
