Ofthe22discrepancies, three(3)weredetermined tohaveexceededtherequirements ofTS3.7.10.Theothernineteen(19)weredetermined tohaveviolatedprocedural requirements.

Nocompensatory actionsweretakenasthisreviewwasofpastevents.Basedonthesetworeviews,areviewwasconducted ofanadditional 10%oftheTSrelatedFWpatrols/tours andothernon-safety relatedFWandFDtoursfortheperiodDecember1993throughFebruary1994.Thisreviewidentified six(6)additional procedural violations andone(1)discrepancy thathadexceededtherequirements ofTS3.7.10.Thediscrepancy involvedtheU-1CDEDGroom.CAUSEOFEVENTForthedocumentation

reviewed, itappearsthecausewasthefailureoftheindividual(s) toperformtheirassignedduties.Acontributing causetotheseeventswasthatmanagement follow-up/monitoring ofactivities didnotidentifythediscrepancies.

ALSISOFEVENTTheTS3.7.10actionstatement foraninoperable fireratedassemblyrequiresthat"...within 1hour:1)verifythatthefiredetectors and/orfiresuppression systemonatleastonesideoftheinoperable assemblyareOPERABLEandestablish anhourlyfirewatchpatrol,or2)establish acontinuous firewatchpatrolononesideofthepenetration...".

FailuretomaintainanhourlyFWpatrolwasaviolation ofTS3.7.10andisreportable under10CFR50.73(a)(2)(i)(B).

2~Thedetection forFZ29-GandFZ-15wasinanoperablestate,intheeventofafire,thedetectors wouldhavealarmedintheU-lControlRoom,whichismanned24hoursaday.Thisalarmcondition wouldhavebeeninvestigated andtheappropriate FireBrigaderesources wouldhavebeendispatched tomitigatethefire.1Manualfireprotection equipment wasreadilyavailable forFireBrigadeuseiftheydeemednecessary.

AlthoughanhourlyFWpatrolwasnotconducted for1hour29minutes,1hour38minutes,1hour29minutesand1hour22minutesrespectively, itisconcluded thatthealanna/detection associated withtheaffectedFZwouldhaveallowedtimelyidentification ofafire.Therefore, theseeventsarenotconsidered tohavecreatedasignificant safetyconcern,nordiditcreateasignificant hazardtothehealthandsafetyofthegeneralpublic.CORRECTIVE ACTION2~3~Arandommonitoring programwasdeveloped toreviewfirewatchtours.Theseevents,lessonslearnedandmanagement expectations wereconveyedtoPlantProtection personnel duringmeetingsheldduringthemonthofMay1994.FireWatchpatroltrainingandimplementing procedures arebeingrevisedtoincorporate thelessonslearnedfromtheseevents.PREVIOUSSIMII.AREVENTS"NoneNRCFono35BA(689)
