NRCFORM388AI4-95)LZCENSEEEVENTREPORT(LER)TEXTCONTINUATIONU8,NUCLEARRMUIATORYCOMMISSIOSt.LucieUnit105000335YEARSEQUENTIALREVISION97-007-02OF4TEXT(ifmorespeceisrequired,useedditionelcopiesofNRCForm366AJI17lOnMay2,1997,St.LucieUnit1wasat100percentpower,andSt.LucieUnit2wasinMode6anddefueledforarefuelingoutage.AnoutageinspectionoftheUnit2ReactorCoolantPumps(RCPs)[EIIS:AB:P]identifiedexternaloilleakagenotcapturedbytheRCPoilcollectionsystem.TheexistingRCPOilCollectionSystemdesignconsistsofthreeoildripenclosuresandfivedrippansoneachreactorcoolantpumpmotor.Agravitydrainpipingsystemisprovidedtotransportanyaccumulatedoilfromthesedevicestoa225gallonoilcollectiontank.Thetankisventedandprovidedwithaflamearrestorwithamagneticliquidlevelgagetoprovidelocalindicationofoilinthetank.Thecollectiondevicesaredesignedtocollectoilleakagefromthefollowingpotentialleakagesources:1.2.3.4,Theliftoilsystem,allflangedconnectionsontheoilcoolers&circulatingoilpiping,oilfillanddrainlines,and,oillevelinstrumentationpiping.TheUnit2RCPinspection'otedoilleakageatboththeupperandlowerRCPmotoroilreservoirlocations.Theinvestigationintothesourceoftheoilfromthelowerreservoiridentifiedminorweepagefromthe"piepan"(loweroilreservoirtopaccesscovers)/RCPshaftsealarea.Becauseofthesmallamountofoilleakage,itisnotpossibletopositivelyconfirmwhetherthesourceisfromtheRCPshaftsealorthe"piepan"covers.TheinvestigationintothesourceofleakagefromtheupperoilreservoiridentifiedaleakingRCPbearingresistancetemperaturedetector(RTD)penetrationfittingonthemotortophat,whichallowedoiltocollectontopoftheRCPmotorhousingandrundownthesideofthemotor.Theseleakagesitesareatatmosphericorlowpressuresandarelocatedabovethenormalupperandlowerreservoiroillevels.However,theselocationscanpotentiallyleaksmallamountsofoilduetotheoilsplashcausedbytherotatingcomponentsinsidetheoilreservoirs.ItwasthereforedeterminedthatthedesignoftheRCPOilCollectionSystemwasdeficientinthatnotallpotentialoilleakagesourceshaveprovisionsforcollection.TheoriginalRCPOilCollectionSystemdesigndidnotconsiderthethesecomponentpartstobepotentialleakagesourcesbecausetheirlocationsareabovethenormaloperatingoilreservoirlevel.Additionally,thelowerreservoiroperatesonlyataslightlypositivepressureandtheupperreservoiroperatesatatmosphericpressure.Thepotentialforleakageduetooilsplashwasnotconsideredintheoriginaldesign.Therefore,nodesignfeatureswereprovidedtocollectoilfromtheselocations.NRCFoRM366AI495I

NRCFORM366AI4-96ILICENSEEEVENTREPORT(LER)TEXTCONTINUATIONU.S.NUCLEARREGULATORYCOMMISSISt.LucieUnit105000335YEARSEQUENTIALREVISION97-007-03OF4TEXT'ilfmorespeceisrequired,useedditionalcopiesofNRCFarm366AiI17)TheRCPOilCollectionSystemdesignisrequiredtomeettherequirementsof10CFR50AppendixRSectionII.O,whichstatesthat"thesystemshallbesodesigned,engineered,andinstalledthatfailurewillnotleadtofireduringnormaloperationordesignbasisconditionsandthatthereisreasonableassurancethatthesystemwillwithstandtheSafeShutdownEarthquake.Suchcollectionsystemsshallbecapableofcollectinglubeoilfromallpotentialpressurizedandunpressurizedleakagesitesinthereactorcoolantlubeoilsystems."However,theSt.LucieRCPOilCollectionSystemdesignisdeficientinthatnotallpotentialoilleakagesiteshaveprovisionsforbeingcollected.Therefore,itwasdeterminedthatthisconditionwasreportablebecausetheRCPOilCollectionSystemwasoutsideoftheAppendixRSectionIII,Odesignbases.BecauseUnit1andUnit2havesimilarRCPmotorsandoilcollectionsystems,thedesigndeficienciesassociatedwiththeupperoilreservoirtophatareaandtheloweroilreservoirshaftseal/"piepan"areaareapplicabletobothUnits1and2.Thefollowingassessmentwasmadetodeterminethefirepotentialresultingfromanyuncollectedoilattheselocations.Theidentifiedleakagesiteswereassessedinordertocharacterizeanypostulatedleakageandresultantfirepotential~Thepotentialleakagesitesareeitherunpressurizedoronlyslightlypressurizedandarelocatedabovethenormaloperatingleveloftheoilreservoirs.Anypostulatedleakagewouldbefromtheoilsplashcausedbytherotatingcomponentsinsidetheoilreservoirs.Therefore,oilescapingfromthereservoirwouldnotbesprayed(atomized),butwouldresultinonlyminordripsofuncollectedoil~Thefirepotentialofsuchleakageisminimal;ifadripweretocomeincontactwithahotsurface(temperatureovertheignitiontemperature)itwouldnotaccumulate;thedropwouldburnbutnotsupportfurthercombustion.Ifitcomesincontactwithacoolsurfaceitcouldlocallyaccumulate,butwouldeventuallymigratetothefloor.Oillandingonthefloorwillbedirectedtofloordrainsbasedonfloorslope.Lubricatingoilontheflooorinthesumpisnotsubjecttoignitionbecausethefloorisavastheatsinkwhichwillmaintaintheoilatalowtemperatureandpreventitfromreachingitsignitionpoint.Therefore,thepotentialforafireresultingfromminoroilleakagefromtheselocationsisnotconsideredcredible.Inaddition,RCPmotorupperandloweroilreservoirlevelsaretrendedandreviewedonamonthlybasistoensurethatRCPlubeoilleaksareidentifiedduringplantoperation.AlthoughchangesinthecontainmenttemperatureandpressurecancausesmallvariationsinindicatedRCPoilreservoirlevel,anyappreciablelosofRCPlubeoilisdetectable.TherehavebeennosafetysignificantRCPlubeoilleaksidentifiedoneitherUnit1or2.NRCFORM368Al4.95I NRCFORM366AI4-95ILICENSEEEVENTREPORT(LER)TEXTCONTINUATIONU.S.NUCLEARRMULATORYCOMMISSIOSt.LucieUnit105000335YEARSEQUENTIALREVISION97-007-04OF4TEXTItfmorespaceisrequired,useadditionalcopiesofNRCForm366AJI17)ContinuedoperationofUnit1isjustifiedbasedontheabovediscussionandthefollowingadditionalinformation.Currently,theSystemEngineerhasconcludedthatnosignificantoilleaksarepresentinanyoftheUnit1RCPs,basedondatacollectedoverthelastsixmonths.Additionally,theinspectionofthe.RCPs,duringtherecentUnit1ShortNoticeOutage(SNO)onApril19,1997,didnotidentifyanysignificantexternaloilleaks.Anoilfilmonthe1A1RCPandplatformwasnoted.Eventhoughtheprecisesourceofthisoilfilmisnotknown,itisevidentthattheexistingminoroilleakagehasnotcomeincontactwithanignitionsourceincontainment.Thus,itisreasonabletoassumethatfurtherleakagefromthesamelocationwouldalsonotcomeincontactwithignitionsources,andthatafireincontainmentfromexistingpotentialleakagesourcesisnotlikely.1~Theuncollectedoilwascleanedup.2.Workorderswereissuedtorepairtheidentifiedleakingpenetrationsandtoresolveleaksonthelowerreservoirs.3.AstepwasaddedtotheRCPGeneralInspectionandRepairMasterPWOtaskdescriptiontoaddressproperinstallationofthelowerreservoir"piepan"covers.4.TheRCPinstrumentationremoval/reinstallationMasterPWOtaskdescriptionwillberevisedtoensureadequateprovisionsforinstallingandsealingpenetrationfittings.5.PCM97-025wasimplementedontheUnit2RCPOilCollectionSystemtoensurethatanyfutureoilleakagefromthemechanicalpenetrationswouldbecapturedbythesystem.6.SimilarmodificationswillbeimplementedontheUnit1RCPOilCollectionSystemduringtheFall1997refuelingoutage.None.NRCFORM368AI4-95I kJ
