4 0 CAUSE DESCRIPTION AND CORRECTIVE ACTIONS 1 Primary cause of this occurrence was determined to be the unplanned RHR system startup. Due to present extended outage the RHR system had not b 2 een maintained filled and a waterhammer effect likely occurred when the RHR system was started. Contributing cause restraint misalignment. Snub ber swivel eye piece replaced. Restraint properly aligned and secured.

DUANE ARNOLD ENERGY CENTER Iowa Electric Light and Power Company LICENSEE EVENT REPORT-Supplemental Data Docket No. 050-0331 Licensee Event Report Date: 790119 Reportable Occurrence No: 78-035 Event Description During a special inspection of RHR piping conducted as.a result of an unplanned RHR system startup, it was discovered that snubber GBB-5-SS-215 had its swivel eye attachment piece bent approximately 5 degrees and restraint GBB-5-SR-230 had its wall attachment plate pulled away from the wall approximately 1/4 inch.

Snubber operability requirements listed in Technical Specifications Section 3.6.H.

At the time of this occurrence the plant was in the cold shutdown mode with all fuel removed from the reactor.

Cause Description The primary cause of this occurrence was determined to be the unplanned RHR system startup. This staYtup -occurred during surveillance testing primarily due to a defective procedure. Due to the present extended outage the RHR system had not been maintained filled and a waterhammer effect likely occurred when the RHR system was started. After this unplanned startup an inspection was made of the RHR system to determine if any damage had;occurred. No significant damage other than that to snubber GBB-5-SS-215 and restraint GBB-5-SR-230 was found. A contributing cause of this occurrence may have been misalignment of restraint GBB-5-SR-230 (which is in the vicinity of snubber GBB-5-SS-215) in that the restraint was not in axial alignment with the horizontal pipe run it is designed to restrain. Both the snubber and restraint were manufactured by the Bergen Paterson Pipe Support Corp.

Corrective Action The snubber swivel eye piece was replaced and the snubber tested for lockup and bleed. The restraint wall attachment'plate was repositioned to bring the restraint in alignment with the horizontal.pipe run and properly secured.
