tuzbinetrip(EXIS/TA-TRB),
controlrodinsertion, feedwater isolation (EXIS/JB),
andautomatic starting(EIZS/JB}
ofthetwoMDAFPFs(EIIS/BA-P}
andtheTDAFP(EIIS/BA-P) occurredasexpected.
Operating personnel concurrently performed theapplicable Emergency Operating Procedures (EOPs')toverifyproperautomatic protection systemresponses andassessappropriate recoveryactionsfortheindicated plantconditions.

ThewestMFWPhadbeenoverhauled duringtherefueling withtheassistance oftheequipment vendor.However,itcannotbedetermined fromrecordsifthedevicehadbeensetasrequiredwiththeMFWPturbineonturninggearandagainatno-loadoperating speedconditions.
Also,eightdaysaftertheinitialU2powerascension following therefueling, theshaftpositionalarm(EIIS/IB-ZA) beganintermittently alarminginthecontrolroom.Thepositiondevicewascheckedlocally(attheturbine)andwasindicating "0LIdeflection.
Otherindications ofbearingproblems, suchasexcessive temperature orvibration, alsodidnotexist(forboththealarmproblemandthelatertrip).Plantmaintenance personnel atthattimetriedtoadjustthedevicewhileitwasinstalled ontheMFWP(withit'stripfunctiondisabled) usingvendorinformation forguidance.
Atthattimethedevicewasleftwithfreetravelinthe"+"and"-"directions ofshaftmotion.Folloving thetrip,thedevicewasfoundwithit'stravelinthe"+"direction entirelyusedup.Theinvestigation ofthedeviceindicated noreasonvhyitwouldsuddenlyshiftfroma"tripfree"toa"hardtrip"condition asalladjustment screwsverefoundtight.Thedeviceisboltedtothethrustbearinghousingwithtwohold-down bolts.Thepost-trip inspection foundsomeforeignmaterialontheslidingshoe.AnalsisofEventThisreportisbeingsubmitted inaccordance with10CFR50.73,paragraph (a)(2)(iv}
asaresultoftheunplanned actuation oftheUnit2RPSandESF.Theautomatic protection functions vereverifiedtohavefunctioned properly.
Theeventdidnotinvolveanunreviewed safetyquestionasdefinedin10CFR50.59paragraph (a)(2),anddidnotadversely impactthehealthandsafetyofthepublic.Corrective ActionTheshaftpositiondevicewasremovedfromtheW-MFWPturbinedriverandinspected.
