==Dear Mr. Rhoades:==

In order for the NRC to adequately prepare for this inspection, please furnish: 1) an index or summary of condition reports written in the past 2 years in which licensed operator errors were determined to be the root or contributing cause; 2) an index or summary of condition reports written in the past 2 years on simulator performance problems; 3) an index or summary of simulator performance test results in the past 2 years; and 4) the examination schedule, sample plan, written examinations, and operating test scheduled for the week of the inspection to the NRC by the week of October 11, 2021. Mr. Swiger has been advised of this request and provided with the name and address of the NRC lead inspector assigned to this inspection.
