(usedtomaintainicecondenser temperature) wereintermittently inoperable formaintenance and/orrepair.Inaddition, astheresultofapersonnel error,mostoftheAHU'swereinoperable foraperiodofaboutfourdaysattheendofJuly;however,ithasbeenconcluded thattheinoperability oftheAHU'sdidnotsignificantly contribute tothefrostandiceformation experienced; 2)duringtheUnitshutdownimmediately preceding thissurveillance, itwasdiscovered thatasecondary sidesteamleakhadoccurredinsidecontainment, possibly'affecting thehumidityintheicecondenser; however,ithasbeenconcluded thatthesteamleakagedidnotsignificantly contribute tothefrostandiceformation experienced, astheformation wasverysimilartothatexperienced inthepast(theonlyapparenteffectofthesteamleakontheicecondenser wasminoramountsoffrost/ice formation ontheintermediate deckdoors).Withtheexception oftheAHU's,therewerenoinoperable structures, components orsystemsthatcontributed tothisevent.CauseofEventItisbelievedthatsublimation oficeorhighhumidityinthecontainment aircouldhavecontributed tothisproblem.Furtherinvestigation ofthisevent.isongoing.AnalsisofEventTheWestinghouse evaluation indicated thatlatti'cefrost/ice formation ofupto20percentofthetotalflowpassageareacouldbepresentwithoutthepeakIceCondenser Compartment Pressureexceeding thedesignlimit.Sincethefrost/ice buildupidentified inBays1,2,3,4,6,8,12,14and15constitute atotalflowblockageareawhichislessthan20percentlimit,thissituation isboundedbytheWestinghouse evaluation.
Ourevaluation indicates thattheamountofflowblockageduetofrostandicebuildupnotedintheIeeCondenser canbetolerated withoutadversely affecting theIceCondenser functionduringaLossofCoolantAccident.
Basedontheaboveinformation andtheWestinghouse evaluation, itisconcluded thattheabnormaldegradation eventdoesnotconstitute anunreviewed safetyquestionasdefinedin10CFR50.59(a)(2),

DescritionofEventTheas-foundvisualinspection oficecondenser (EIIS/COND) flowpassagesconducted onSeptember 8,1988,identified frostandiceaccumulation greaterthan3/8inchintwoflowpassagesinBay1,twoflowpassagesinBay2,fourflowpassagesinBay3,twoflowpassagesinBay4,fourflowpassagesinBay6,twoflowpassagesinBay8,twoflowpassagesinBay12,twoflowpassagesinBay'14andtwoflowpassagesinBay15.Subsequently, theinspection wasexpandedtoincludeatleasttwentyadditional flowpassagesineachaffectedbay.Thisinspection revealedanadditional twoflowpassagesinBay2,sixflowpassagesinBay3andsixflowpassagesinBay4withmorethan3/8inchfrostandicebuildup.Atotalofthirty-six flowpassageswereaffected.

Thereareatotalof3072flowpassagesintheIceCondenser.

Attachments 1through11graphically describethegeometryoftheflowpassagesandthelocationoftheice/frost accumulation.

Technical Specification (T/S)4.6.5.l.b.3 requiresthattheicecondenser bedetermined operableatleastonceper9monthsbyverifying, viavisualinspection ofatleasttwoflowpassagespericecondenser bay,thataccumulation offrostor.iceonflowpassagesbetweenicebaskets(EIIS/COND-BSKT),

pastlatticeframes(EIIS/COND-FRM),

throughtheintermediate andtopdeckfloorgrating,orpastthelowerinletplenumssupportstructures (EIIS/COND-SPT) andturningyanesisrestricted toanominalthickness of3/8inch.Ifoneflowpassageperbay-isfoundtohaveanaccumulation offrostoricegreaterthanthisthickness, arepresentative sampleoftwentyadditional flowpassagesfromthesamebayshallbevisuallyinspected.

Iftheseadditional flowpassagesarefoundacceptable, thesurveillance, programmayproceedconsidering thesingledeficiency asuniqueandacceptable.

Morethanonerestricted flowpassageperbayisevidenceofabnormaldegradation-oftheicecondenser.

1)duringthesurveillance intervalpriortotheSeptember 8,1988test,severalofthe60airhandlingunits(AHU)(EIIS/AHU)

(usedtomaintainicecondenser temperature) wereintermittently inoperable formaintenance and/orrepair.Inaddition, astheresultofapersonnel error,mostoftheAHU'swereinoperable foraperiodofaboutfourdaysattheendofJuly;however,ithasbeenconcluded thattheinoperability oftheAHU'sdidnotsignificantly contribute tothefrostandiceformation experienced; 2)duringtheUnitshutdownimmediately preceding thissurveillance, itwasdiscovered thatasecondary sidesteamleakhadoccurredinsidecontainment, possibly'affecting thehumidityintheicecondenser; however,ithasbeenconcluded thatthesteamleakagedidnotsignificantly contribute tothefrostandiceformation experienced, astheformation wasverysimilartothatexperienced inthepast(theonlyapparenteffectofthesteamleakontheicecondenser wasminoramountsoffrost/ice formation ontheintermediate deckdoors).Withtheexception oftheAHU's,therewerenoinoperable structures, components orsystemsthatcontributed tothisevent.CauseofEventItisbelievedthatsublimation oficeorhighhumidityinthecontainment aircouldhavecontributed tothisproblem.Furtherinvestigation ofthisevent.isongoing.AnalsisofEventTheWestinghouse evaluation indicated thatlatti'cefrost/ice formation ofupto20percentofthetotalflowpassageareacouldbepresentwithoutthepeakIceCondenser Compartment Pressureexceeding thedesignlimit.Sincethefrost/ice buildupidentified inBays1,2,3,4,6,8,12,14and15constitute atotalflowblockageareawhichislessthan20percentlimit,thissituation isboundedbytheWestinghouse evaluation.

Ourevaluation indicates thattheamountofflowblockageduetofrostandicebuildupnotedintheIeeCondenser canbetolerated withoutadversely affecting theIceCondenser functionduringaLossofCoolantAccident.

Basedontheaboveinformation andtheWestinghouse evaluation, itisconcluded thattheabnormaldegradation eventdoesnotconstitute anunreviewed safetyquestionasdefinedin10CFR50.59(a)(2),

nordoesitadversely impacthealthandsafety.Though-ourevaluation hasconcluded thatthedegradation isnotserious,webelieveissuanceofthisvoluntary LERisappropriate sincesomedegradation hasbeenidentified.
