One alignment (jacking) bolt on the southeastcornerof P202E was missing from the motor. This screw has no function in the operation of the equipment after installation,so this is not consideredan adverse seismic condition.
Three alignment (jacking)bolts were observed to be in contact with the motor housing which is contraryto standardpractice of backing out the alignment bolts after alignment is achieved and primary mounting hardwareis secured. This condition was previously identified by CR-PNP-2011-3626.Work Order 286700 is scheduled to correct this condition. See attachedphoto.

The tag number on P202E was missing. Operationswas notified, and an ESOMS label request was initiated.See CR-PNP-2012-044 11. The equipment was verified for the walkdown using auxiliary equipment tags (see attachedphoto).

Anchorage
- 1. Is the anchorage configuration verification required (i.e., is the item one Y[-] NO*
of the 50% of SWEL items requiring such verification)?
The SEWs package for this component conservatively evaluates one of four various instrumentrack anchorages. This rack was not evaluated, and therefore configurationcan not be verified by the package. All bolts are visible, so the remainder of this section is applicable.

- 4. Is the anchorage free of visible cracks in the concrete near the YN NDI U-- N/AD1 anchors?

C2205A is bolted to C2205B, so this is not a seismic interaction concern. C2205A is also adjacent to but not bolted to A TWS rack 2275; however, there is sufficient clearance( > 2") between the racks.
Therefore, this is not considered a potentiallyadverse seismic interaction.
One set screw is missing on the fence enclosing the racks. This is not a seismic hazard since the fence is judged to still be securely mounted to the floor, but CR-PNP-2012-04659was written and WR 287549 initiated to address the concern. See attachedphoto.

Anchorage
- 1. Is the anchorage configuration verification required (i.e., is the item one Y[-] NZ of the 50% of SWEL items requiring such verification)?
The SEWs package conservatively evaluates similar rack C2206B, which has only six anchors, and comparatively accepts the eight anchorageconfigurationfor rack C2206A. Therefore, since a drawing does not specify the eight bolt configuration, this component was not chosen for the 50% verification.

Racks C2206A and C2206B are bolted together and therefore do not pose a seismic risk to each other.
The robust barrierfence aroundRacks C2206A & C2206B has set screws missing from the base brackets of three of the verticalposts.
The fence consists of an interconnectedframework of light support members and wire mesh. The weight and mass of the assembly is relatively low. The remainingposts (approx 5) have the set screws installed. The base brackets with the missing set screws still function to resist lateralloads. Since vertical seismic accelerationsare less than Ig (Ref Spec C114ERQ), there is no expected uplift at the post locations.
Based on these considerations, the identified condition does not representa potential adverse seismic interaction. CR-PNP-2012-04627 was written to replace missing screws.
- 8. Are overhead equipment, distribution systems, ceiling tiles and lighting, YZ NDl UD- N/AD-'
and masonry block walls not likely to collapse onto the equipment?
The west masonry block walls next to the rack, 65.9 and 65.10 are safety related.

The cover for pressure transmitter36-PT-654 is missing one of four bolts which keep the cover secured to the equipment. This is not a seismic concern due the redundancy of the other three bolts. CR-PNP-2012-04629 was written to address this item and calls for replacement of the missing fastener.
