hiEMO FOR FACSIMILE TRANSMISSION TO CONFIRM PROMPT REPORTABLE OCCURRENCE 335-80-4 IMPROPER ELECTRICAL LINEUP This memo is to confirm the telephone notification made to Mr. Martin of your office on 1/24/80.

At about 3:00 PM on 1/24/80 it was noted that a feeder breaker (2B4-5) from the B Startup transformer to the B 4160V vital bus was open. This condition had existed since 5:00 PM on 1/18/80 and was not in compliance with T.S. 3.8.1.1. Action a. regarding offsite A.C. power supplies which require testing of the "B" Diesel Generator (D/G) every 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br /> when the "B" Startup transformer is out of service and limits the out of service period to 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />. Additionally for 5-1/2 hours on 1/24/80 the A D/G was taken out of service for periodic maintenance. The sur-veillance requirements and time limits of T.S. 3.8.1.1. Action b. were met. Also the B D/G was tested satisfactorily on 1/18/80 and again on 1/24/80 so it was operable during the period in question as was the "A" "A" D/G.

Startup transformer, and (except for the period on 1/24/80)

The breaker was closed by 3:15 PM on 1/24/80. The apparent cause is an improperly labeled breaker. For interim protection until this matter has been thoroughly investigated, the A and B breaker cubicles have been locked and are being surveilled daily. (The breakers were checked every 2 hours2.314815e-5 days <br />5.555556e-4 hours <br />3.306878e-6 weeks <br />7.61e-7 months <br /> until this action was completed.) Also, no switching orders on this system will be performed until we are sure the problem has been thoroughly investigated and is fully understood. Investigation and eval-uation will be continued and final results including further corrective action will be reported.
