11. Abstract of Examinations:

An inservice examination (ISi) of selected Class 1, Class 2 and Class 3 components of Public Service Electric and Gas Company's (PSE&G) Salem Generating Station, Unit 1, was performed by Public Service Electric & Gas (PSE&G), Magnaflux Quality Services (MQS), Southwest Research Institute (SwRI), Virginia Corporation of Richmond (VCR) and AEA Technology Engineering Services (AEA) personnel during the April 1995 - April 1998. These examinations-constituted the twelfth (2nd Period, 3rd Outage) at Salem 1 during the Second 10-year Inspection Interval of operation.

Exams were performed in accordance with the 1983 Edition through Summer 1993 Addenda of ASME Section XI and the Salem lnservice Inspection Program Long Term Plan.

ISi was performed utilizing visual (VT), magnetic particle (MT), liquid penetrant (PT), manual ultrasonic (UT), mechanized ultrasonic (M-UT) and radiography (RT) nondestructive examination techniques.

This report also contains augmented examinations required by the Salem Technical Specifications, Regulatory Guides, Circulars and Bulletins issued by the United States Nuclear Regulatory Commission.

11.2 ISi Section XI Visual examinations on valves and bolting PSE&G's ISi Group conducted eight Class 1 bolting exams and one Class 1 valve internal exam.

ISi Section XI examinations were conducted on the*following systems:

Safety Injection Residual Heat Removal The following was identified during the course of these examinations:

Valve 1RH26 (WinlSI Sum# 193800) had a visual VT-3 internal examination, which revealed seating surface wear. The valve interior was resurfaced and an acceptable post rework VT-3 valve internal examination was performed. An additional sample was not required, since the valve is the only one identified in that valve group.

There were no other unacceptable indications identified.

11.3 ISi Section XI Visual examination of components supports PSE&G conducted four Class 1, 2 & 3 component support visual VT-3 and VT-3/4 examinations.

ISi Section XI examinations were conducted on the following systems:

Main Steam Safety Injection Reactor Coolant Chemical and Volume Control There were no unacceptable indications identified.

11.4 ISi Section XI System Leakage Examinations PSE&G's ISi Group conducted four System Pressure Tests during a System Functional or lnservice Test, on Nuclear Class 1, 2 and 3, in accordance with ASME Section XI.

A system Leakage exam was performed in accordance with ASME Section XI on all Class 1 Systems. The system leakage test was performed with the Reactor Coolant System in Mode 3 (normal operating temperature and pressure) with the insulation installed. No pressure boundary leakage was detected.

Work requests were initiated to correct minor mechanical leakage that was not corrected during the system walkdowns.

Class 2 and 3 System Pressure Tests were conducted on the following systems:

- Auxiliary Feedwater - Sampling 11.5 RCP Flywheel Augmented Examinations per USNRC Regulatory Guide 1.14 SwRI under the direction of the ISi Group performed a Ten year exam (with the Flywheels removed from the motors) on all Salem Unit 1 & 2's RCP Flywheels at the Westinghouse Cheswick, PA facility as identified below by the Flywheel serial number:

11.6 NRC Bulletin 88-08 "Thermal Stresses in Piping Connected to the Reactor Coolant System" Augmented Examinations VCR under the direction of PSE&G's ISi Group performed augmented ultrasonic and dye penetrant examinations on a total of 20 components, broken down as follows:

11.7 Augmented Exams per Branch Technical Position MEB 3-1 30 welds were volumetrically examined, to comply with the augmented requirements of NUREG 0800 Standard Review Plan, Section 3.6.2, Branch Technical Position MEB 3-1.

The UT was performed by VCR with no unacceptable indications identified.

11.8 NRC Bulletin 88-11 "Pressurizer Surge Line Thermal Stratification" Augmented Examinations PSE&G's ISi Group performed visual examinations on all existing surge line pipe supports I HEBA restraints and their associated insulation interfaces.

A total of ten supports were examined.

There were no unacceptable indications identified.

11.10 PSE&G's Supplemental Ultrasonic Examinations on the old Steam Generator Feedwater Nozzle to Forging Expanders AEA ultrasonically examined the four Steam Generator Feedwater Nozzle inner radius and the barrel of the nozzle including the Feedwater thermal sleeves.

There were no unacceptable indications identified.

11.11 PSE&G Augmented Visual Examinations on the Reactor Coolant Pump Main Flange connection (R/T# 290049)

PSE&G's ISi Group performed a visual examination of the Reactor Coolant Pumps Main Flange connection for evidence of leakage in accordance with R/T# 290049.

Each of the four RCP's showed evidence of RCS leakage (dried boron on the main flange connection area and the bolting). All the RCP main flange studs were removed, several at a time, and visually VT-1 examined for damage and reinstalled and re-torqued. Several studs were found with unacceptable conditions and were replaced as required (See the following preservice examination descriptions for specific details)

11.20 Technical Specification 4.7.9 (c) Functional Testing of Selected Hydraulic and Mechanical Snubbers HYDRAULIC SNUBBERS (FIRST TEST SAMPLE)

Hydraulic snubbers were tested using the Technical Specification 47.9(c),

10% sample plan by Wyle Laboratories using the API in-place snubber test machine and the Wyle Laboratories model 150 snubber test machine as follows: - Two SIG 1,000 Kip Rexnords

- One MSIV 200 Kip Paul Monroe

- One Lisega small hydraulic snubber All hydraulic snubbers tested satisfactorily, except for the one Lisega small snubber.

MECHANICAL SNUBBERS (FIRST TEST SAMPLE)

Mechanical snubbers were tested using the Technical Specification 4.7.9(c), 10% sample plan using the Wyle Laboratories model 150 snubber test machine as follows:

- Two Mechanical large (PSA-35) 11 mechanical medium (PSA-1, 3 and 10)

- Four mechanical small (PSA-1/4 and 1/2)

All mechanical snubbers tested satisfactorily, except for the two mechanical medium (PSA-1, 3 and 10) snubbers.

HYDRAULIC AND MECHANICAL EXPANDED SNUBBER TEST SAMPLES Due to Hydraulic and Mechanical snubbers test failures the following expanded snubber test samples were tested using the Wyle Laboratories model 150 snubber test machine.

Four snubbers (100% of the Lisega hydraulic snubbers)

- Two expanded samples totaling 22 mechanical medium (PSA-1, 3 and 10)

PSE&G Engineering performed a stress evaluation on all failed snubber locations and concluded that the piping and components to which the snubbers were attached were not impaired due to in-operability of the snubbers.

STEAM GENERATOR REPLACEMENT, MSIV REFURBISHMENT &

MISC.

Sixteen SIG snubbers were removed during the SIG replacement and were returned their position, two SIG snubbers were retested satisfactorily, eight MSIV snubbers were removed for MSIV rebuild and were restored and one retested satisfactorily. Five none Code pre-NF snubbers were found in the Turbine area and tested and were replaced with new snubbers.

HYDRAULIC AND MECHANICAL SNUBBER EXTENDED OUTAGE SAMPLE Due to the extended outage greater than 18 months, the following hydraulic and mechanical snubbers were tested to comply with Technical Specification 4.7.9 (c) (10% sample):

- One 1,000 Kip Paul Monroe

- One 1,000 Kip Rexnord

- One 200 Kip Paul Monroe snubbers

- One mechanical large (PSA-35) 11 mechanical medium (PSA-1, 3 and 10) 100% mechanical small (PSA-114 and 112) under a new random sample were previously tested satisfactorily. Thus, no additional testing was required.
