Furthermore, inaccordance withUnit2Technical Specification 4.8.1.2.2, theeventisbeingsubmitted asaSpecialReport.At1119onMarch15,1989,the2Bdieselgenerator wassuccessfully restarted forthemonthlysurveillance runafterresetting thehighcrankcase pressuretripfromthepreviousstartattempt.Thedieselgenerator outputbreakerwouldnotclose.The2Bdieselgenerator wassecuredanddeclaredoutofservice.Thefailureofthedieselgenerator outputbreakertoclosewasduetoafaultyVoltageMonitoring Relay(VMR)intheDieselCenerator ControlPanel.TheVMRisoneofthepermissive contactsthathastobemadeupbeforetheoutputbreakercanbeclosed.Thisrelayprevented loadingofthedieselgenerator.

Thiseventwasdetermined tobeavalidfailureinaccordance withReg.Cuide1.108,SectionC.2.e.Corrective actionstakenwerethereplacement oftheVoltageMonitoring Relayandcompletion oftherequiredsurveillance toreturnthedieselbacktoservice.Additional information:

a)ThisfailureisthesecondvalidfailureinthelastI00validtests.b)Thedieselwasoutofserviceforthiseventfor8hrs.and12min.c)Thecurrentsurveillance intervalisonceevery31days.

SPECIALREPORTDIESELGENERATOR FAILURESPageThree2BDIESELGENERATOR FAILURE:3/21/89Inaccordance withNRCReg.Guide1.108,SectionC.2.e,thiseventisconsidered anon-valid dieselgenerator failure,sinceitwascausedbya"malfunction ofequipment thatisnotoperative intheemergency operating mode."However,inaccordance withUnit2Technical Specification 4.8.1.2.2, thiseventisbeingsubmitted asaSpecialReport.At0729onMarch21,1989,the2Bdieselgenerator automatically startedduetotheinadvertent actuation oftheContainment Isolation Actuation Signal(seeLER389-89-003).

WhentheContainment Isolation Actuation Signalwasreset,the2Bdieselgenerator trippedonhighcrankcase pressure.

Thiseventoccurredpriortotheinstallation oftheoilsplashguardonthe2Bdieselgenerator.

The2Bdieselgenerator wasplacedoutofservicetocorrecttheproblem.Thecauseofthehighcrankcase pressuretripisinthedesignofthe12cylinderengine.Undercertainconditions, (e.g.,faststartsand/orlowerlubeoiltemperatures),

thelubeoilreliefvalvewillrelievelubeoilincloseproximity tothe'crankcase pressuredetector, thuscausingahighcrankcase pressuretrip.Thehighcrankcase pressuretripisnotoperative whenthedieselgenerator isintheemergency operating mode;therefore, thisisconsidered anon-valid failure.Corrective actionstakenweretheinstallation ofoilsplashguardsonthe2Aand2B12cylinderengines.Also,sincetherecentlyissuedAmendment No.39totheTechnical Specification-allows idlestarts(450rpm'ice900rpm),procedure changesarebeingpreparedinordertoperformidledieselenginestartswhenthesoakbackoilsystemhasbeenoutofserviceforextendedperiods.Additional information:

a)Twovalidfailuresinthelast100validtestsb)Thedieselwasoutofserviceforthiseventfor5hrs.and55min.c)Thecurrentsurveillance intervalisonceevery31days.
