Themostprobablerootcauseofthemispositioned valvewaspersonnel errorbyautilitynon-licensedoperator, thoughtheresponsible individual hasnotbeenidentified.

Whileperforming thisvalvestatuscheck,the1Bdieselfueloiltransferpumpdischarge valve(EIIS:ISV) wasfoundinthelockedclosedposition.

Thisadministratively renderedthe1Bdieselgenerator outofservice,perTechnical Specification 3.8.1.1.b.3.

Thevalvewasunlocked, re-opened, lockedopened,andindependently verifiedlockedopen,within5minutesoffindingitclosed.Priortothisevent,thelasttimethepumpdischaxge valvewasoperatedwasat1015onJune5,1989,toplacethe1Bdieselfueloiltankonrecirculation.

Thiswasrequiredforachemistry sampleofthe1Bdieselfueloiltank.Althoughthevalvewasre-openedandlockedopenafterthesamplewascompleted, theclosingandre-opening ofthevalvewasnotrecordedintheValveDeviation Loginaccordance withAdministrative Procedure 1-0010123.

Thevalve'slockedopenpositionwasverifiedindependently onJune7byautilitynon-licensed

operator, andonJune13byaQCInspector, whenthevalvestatuscheckofAdministrative Procedure 1-0020235A wasperformed.

OnJune14,1989,thevalvewasfoundinthelockedclosedposition.

CAUSEOFTHEVENTNofueloilsampleswexetakenandnomaintenance wasperformed onthedieselfueloilsystembetween1600onJune13,1989and1740onJune14,1989.OnlyOperations Department personnel haveaccesstothevalvelockingdevicekey.Therefore, themostprobablerootcauseoftheeventwaspersonnel errorbyautilitynon-licensed operator.

Afterinvestigation, theresponsible individual hasnotbeenidentified.

anyoperation orcondition prohibited bytheplant'sTechnical Specifications.

Sincethe1BDieselwasadministratively outofserviceforaslongas25hours,thesurveillance requirements ofTechnical Specification 3.8.1.1.,

whichrequiresthattheoperability oftheotherDieselbetestedwithin1hourandevery8hoursthereafter, shouldhavebeenperformed.

However,thissurveillance wasnotdonebecausetheoperators werenotawareoftheoutofservicediesel.Eachemergency dieselisdesignedforfullpost-accident loadoperation for75minutesonthedaytanks.Thefueloiltransferpumpstartsandtheinletvalvetothedaytanksopensatadaytanklevelof23inches.Withthefueloiltransferpumpdischarge valveshut,thedieselgenerator wouldstartifrequired.

However,automatic makeuptothedaytankwouldbeprevented, andthedaytanklevelwoulddecrease.

Approximately 39minuteslater,atalevelof11inchesinthedaytank,thelowlevelannunciator wouldalerttheoperators oftheproblemlocallyandinthecontrolroom.Thiswouldleaveapproximately 36minutestodispatchanoperatortotheareatolocate,unlock,andre-openthepumpdischarge valve.Sincetheemergency dieselwasavailable toperformitssafetyfunction, thehealthandsafetyofthepublicwerenotaffectedbythisevent.CORRECTIVE ACTIONS1)Operations personnel re-opened andlockedopenthevalve.2)TheOperations Supervisor counselled theoperators ontheimportance of,andtheuseof,thevalvedeviation log.ADDITIONAL NFORMAON1.COMPONENT IDENTIFICATION 2~Thiseventwasnotcausedbycomponent failure.PREVIOUSSIMILAREVENTSForasimilarevent,seeLER5335-82-48, whichpertainstotheinoperability ofthe1Adieselfueloilpumpduetovalvemispositioning becauseofimproperrestoration frommaintenance.
