revision, itwasidentified thatforshortperiodsoftime(typically twotofiveminutes)whilearadiation protection technician waschangingouttheparticulate filtersandiodinecartridges forthelowercontainment radiation monitors(ERS-1300 andERS-1400inUnit1andERS-2300andERS-2400inUnit2)(EIIS/IL-MON) anopenpathwayfromthecontainment atmosphere totheauxiliary buildingwasestablished.

Theparticulate filterandiodinecartridge changeoutwasperformed peranapprovedprocedure withtheradiation protection technician presentthroughtheentirechangeoutprocedure.

Thechangeoutisperformed atleastonceperday,,andthesameprocedure hasbeenfollowedsincethesemonitorswereinstalled onUnit1andonUnit2in1985.Thecondition isuniquetothecontainment integrity requirements duringcorealterations due-tothefactthatthereisnoautomatic containment isolation signaloperablefortheradiation monitoring systemduringthismodeofoperation.

Sincethiscondition wasnotrecognized duringpreviousrefueling outages,itisverylikelythattheopenpathwaymayhaveexistedduringpreviouscorealterations ormovementofirradiated fuelwithinthecontainment building.

Noinoperable structures, components, orsystemscontributed tothesignificance ofthisevent.CauseofEventThiseventwascausedbytheprocedural inadequacy whichallowedtheparticulate filtersandiodinecartridges tobechangedduringcorealteration conditions withoutfirstestablishing positivecontainment isolation.

AnalsisoftheEventThiseventisconsidered afailuretocomplywithTechnical Specification 3.9.4ActionStatement requirements inthatcorealterations ormovementofirradiated fuelwithinthecontainment buildingwerenotsuspended whiletheparticulate filtersandiodinecartridges werebeingchangedandtherefore reportable under10CFR50.73(a)(2)(i).
