Thewrittenjoborderactivities usedtocontrolshutdownchemistry samplingwerecorrected toremovereferences totheSpentFuelPoolasanalternate RCSsamplepoint.Ineachidentified casewhenanalyseswerenotdone,theaffectedunitwasatlowpressureandtemperature, andanalysescompleted priortoandafterthemissedanalysesprovidednoindication ofanout-of-limit condition.

However,althoughtheTSCwascancelled, writtendirection basedupontheTSC,whichhadbeenpreviously established bythechemistry department, remainedinplace.Thisdirection statedthatwhenfuelwasremovedfromtheRCS,theSpentFuelPool(SFP)becametheRCSforchemistry samplingpurposes.

ThustheTSsurveillance continued tobeperformed improperly, withtheSFPsampledandanalyzedinsteadoftheRCS.CauseOfTheEventTherootcausesforthiseventinclude:1)Ineffective management ofTSbyLicensing andPlantmanagement, demonstrated bythefactthatNuclearLicensing/Plant Management approvedTSclarification letterscontaining inappropriate TSexceptions; 2)Ineffective ownership ofdepartment performance byChemistry management, demonstated bytheineffective establishment, communication andenforcement ofappropriate standards andalackofawareness ofandinvolvement indepartment activities.

Contributing factorstothiseventinclude:1)therelativedifficulty inobtaining representative RCSsampleswhendefueled, drainedtohalfloopconditions withnorecirculation flowand2)ahistoryofconfusing samplingrequirements andrelated'interpretations fortheRCSinthedefueledcondition.

InJanuary1992,this.culminated inTSC¹54whichaddressed thoseTechnical Specifications applicable toconditions whennofuelwasinthereactorpressurevesselorrefueling canal.TSC¹54referenced twoearlierTSclarification letterswrittenin1989and1990,whichweie"tobeusedtodetermine technical specification applicability withnofuelinthereactorvesselorrefueling canal."UseoftheunclearguidanceinTSC¹54byChemistry Department personnel toimplement RCSsamplingandtestingrequirements contributed tothedescribed TSnoncompliances.

AnalsisoftheEventTechnical Specification (TS)LimitingCondition forOperation (LCO)3.4.7requirestheReactorCoolantSystem(RCS)chemistry tobemaintained withinthelimitsspecified InTable3.4-1.Table3.4-1specifies steadystateandtransient limitsforRCSdissolved oxygen,chlorideandfluorideconcentrations.

ThebasisfortheTSchlorideandfluoridelimitsistoensurethatcorrosion oftheRCSisminimized andreducethepotential forRCSleakageorfailureduetostresscorrosion.

Stresscrackingcorrosion requiresseveralconditions.

AsthefluorideandchloridelimitsintheRCSbeforeandafterdefueledperiodswerelessthan0.02ppm,weilbelowthe0.15ppmlimits,temperatures werewellbelow200'F,andthesystemwasdepressurized, itwasunlikelytheRCSintegrity wascompromised, duetocorrosion.

Itisbelievedthatthiseventresultedinnoequipment damage.Based'upon thisinformation, therewereminimalimplications tothehealthandsafetyofthepublicasaconsequence ofthisevent.CORRECTIVE ACTIONSChemistry personnel havebeeninstructed ontherequirement tofollowtheTSaswrittenwithoutrelianceontheuseofTSclarifications orinterpretations.
