Ofthe22discrepancies, three(3)weredetermined tohaveexceededtherequirements ofTS3.7.10.Theothernineteen(19)weredetermined tohaveviolatedprocedural requirements.
Nocompensatory actionsweretakenasthisreviewwasofpastevents.Basedonthesetworeviews,areviewwasconducted ofanadditional 10%oftheTSrelatedFWpatrols/tours andothernon-safety relatedFWandFDtoursfortheperiodDecember1993throughFebruary1994.Thisreviewidentified six(6)additional procedural violations andone(1)discrepancy thathadexceededtherequirements ofTS3.7.10.Thediscrepancy involvedtheU-1CDEDGroom.CAUSEOFEVENTForthedocumentation
- reviewed, itappearsthecausewasthefailureoftheindividual(s) toperformtheirassignedduties.Acontributing causetotheseeventswasthatmanagement follow-up/monitoring ofactivities didnotidentifythediscrepancies.
