-3>>CORRECTIVEACTIONSThe100%inspectionprogramforPGCCcableconnectorsandrepair/reworkfortheproblemslistedinNCR-3231isnearlycomplete....."='.opreventrecur'rencesoflikepoh's~".:t.~;ei'GCCcoucheconnectors',andwhicharealsoapplicabletotheSusquehannaPGCC/ACRUnit2cables,areindicatedbyGEtobe:t(1)Appropriatetrainingofpersonnelinvolved.(2)'Revisionoffabricationtechniques.(3)Increased/revisedinspectionactivities.(4)GEhasdevelopedasupplementalreinspectioninstruction(GEInspectionInstructionCA-006)toidentifydamagedornonconformingconnectorspriortoshipment.SatisfactorycompletionofthiscablereinspectiononSusquehannaUnit2cableswasreverifiedbyPPRLauditonSept.9-11,1980attheGEmanufacturingfacilityinSanJose,CA.Inaddition,faultycrimpingtoolswererepairedorreplacedandperiodictestingofcrimpingtoolswasincreased.Constructionsitepersonnelhavebeengivenfurtherinstructionsinthehandlingandinstallationofcables.Thesiteelectricianshavebeeninstructedtouseasteadypull(notayank)whenmakingpulltestsandthespringtestersarecalibratedevery30daysorsoonerifdeemednecessary.CONCLUSIONSTherepair/reworkandtestingoftheUnit1PGCC/ACRcablesisnearlycomplete.AllUnit1PGCC/ACRcableswillbecompletedpriortofuelload.TheUnit2PGCC/ACRcableswillbeinstalledandrepairedorreworkedasnecessary.DiscrepancieswillbedocumentedonanNCR.ForUnit2PGCC/ACRcables,fieldinspectionsoftheequipmentandadditionalGEfabricationpersonneltraining,inspectionandrevisedfabricationtechniquesshouldprecluderecurrenceofsimilarproblems.
