Response

Revised the pedigree of the two distractors dealing with condenser overpressurization to state the following: Part two is plausible because 2 F 2 B E overpressurization of the main condenser is a concern at the plant, which is why the condenser has atmospheric relief diaphragms installed to protect it. If too much steam is supplied to the main condenser, it could result in an increase in Main Condenser pressure and cause these diaphragms to rupture. The steam from the extraction check valves, in the example given, is sent to the main condenser, after going through the turbine, but is too small to have a direct impact on main condenser pressure.

NRC: Updated question is SAT.

Refer to Section D of ES-401 and Appendix B for additional information regarding each of the following concepts.

1. Enter the level of knowledge (LOK) of each question as either (F)undamental or (H)igher cognitive level.
2. Enter the level of difficulty (LOD) of each question using a 1 (easy) to 5 (difficult); questions with a difficulty between 2 and 4 are acceptable.
3. Check the appropriate box if a psychometric flaw is identified:

$ Stem Focus: The stem lacks sufficient focus to elicit the correct answer (e.g., unclear intent, more information is needed, or too much needless information).

$ Cues: The stem or distractors contain cues (i.e., clues, specific determiners, phrasing, length).

$ T/F: The answer choices are a collection of unrelated true/false statements.

$ Cred. Dist>: The distractors are not credible; single implausible distractors should be repaired, more than one is unacceptable.

$ Partial: One or more distractors are partially correct (e.g., if the applicant can make unstated assumptions that are not contradicted by stem).

4. Check the appropriate box if a job content error is identified:

$ Job Link: The question is not linked to the job requirements (i.e., the question has a valid K/A but, as written, is not operational in content).

$ Minutia: The question requires the recall of knowledge that is too specific for the closed-reference test mode (i.e., it is not required to be known from memory).

$ #/Units: The question contains data with an unrealistic level of accuracy or inconsistent units (e.g., panel meter in percent with question in gallons).

$ Backward: The question requires reverse logic or application compared to the job requirements.

5. Check questions that are sampled for conformance with the approved K/A and those that are designated SRO-only. (K/A and license-level mismatches are unacceptable).
6. Enter question source: (B)ank, (M)odified, or (N)ew. Verify that (M)odified questions meet criteria of ES-401 Section D.2.f.
7. Based on the reviewers judgment, is the question, as written, (U)nsatisfactory (requiring repair or replacement), in need of (E)ditorial enhancement, or (S)atisfactory?
8. At a minimum, explain any U status ratings (e.g., how the Appendix B psychometric attributes are not being met).

NRC: Bolding NO, and having it appear in two (correct or partially) distractors could be considered cueing. Consider using another incorrect value for distractor A and remove the bolding from the correct response.

Response

Removed the BOLD from NO in the answers A and B, but did not otherwise 11 H 3  ? N E change the answer for distractor A, since a review of the question shows that changing the value of boron being injected in part 1 of distractor A would invalidate part 2 of that answer. This is because the decision to inject with Alternate SBLC would only valid if boron is NOT injecting.

Consider addressing whether the range needs to be changed at this point to help eliminate the need to repeat these recorder readings.

Response

At Dresden, changes to the IRM range switch cause a shift in both the displayed range and (for even to odd range switch position changes ONLY) a change in the displayed value of the reading. For change from an odd range switch position to an even range switch position, the value of the reading remains the same, but the indication range changes. For example, if the range switch were on range 1, with a 46 H 2 X B E value of 35 indicated, the displayed value would be shown on a 0-40 scale.

Changing the IRM range switch to position 2 would still result in 35 being indicated, but it would be displayed on a 0-125 scale. When the IRM switch was taken from range 2 (even position) to range 3 (odd position) the value of 35 would then change to 3.5 and would be indicated on a 0-40 indicated range.

Therefore, it is plausible that the indicated value of 70 in answer B and D could remain the same (the answer would be correct if the range switch were changing from an odd position to an even position. The wording in the first paragraph (above) was added to the pedigree section to enhance the explanation of the question.

Response

Added the following K/A Justification: The shutdown cooling system is designed to be able to line it up to the fuel pool in order to augment the fuel pool cooling system.

This requires installing a spool piece, and physically separating the SDC suction and 60 H 2  ? B E discharge of the SDC pump aligned to the fuel pool from the normal SDC flow path.

The operator must recognize that although 3B SDC pump is aligned and running to the fuel pool, the high recirc loop temperature condition listed in the stem will NOT cause a trip of the 3B SDC pump; and therefore there is no impact to the fuel pool.

The appropriate action, based on the stem of the question, is to address the loss of decay heat removal from the RPV using DOA 1000-01.

DOA 1001-01 does address potential actions regarding use of SDC pumps for fuel pool cooling. For example, it allows redirecting SDC pumps that are aligned to the fuel pool, to the RPV if needed. In this case, re-aligning the pump will not accomplish anything, since it would simply trip also. The cause effect relationship is not directly Page 10 of 21

NRC: 1) The stem, as written, needs correction - additional of amount time should read additional time. 2) Spell out what TCCP means on it first use. 3) The Answer Explanation needs to be clarified to ensure that the correct answer is the only correct answer (there are exceptions given that must be considered). 4) SRO Only Criteria needs to be spelled out in the pedigree. 5) Explain why this question should be considered significantly modified - the correct answer is essentially the same.

Response

1. Changed that portion of the stem to say, Assuming the reactor remains at power, this jumper may remain installed a MAXIMUM of __________

additional days before a 10CFR50.59 review MUST be completed.

2. Spelled out TCCP in first use.

76 F 2  ? M E 3. Added to question that reactor remains at power. Added direct to bullet two, to clarify that is it is in direct support of maintenance. Since the jumper is listed as being in direct support of maintenance, and the reactor is at power, the maximum time it can remain installed is 90 days (15 days +

75 days = 90 days),

4. Pedigree contains SRO CRITERIA: 10CFR55.43(b)(3) - Facility licensee procedures required to obtain authority for design and operating changes in the facility.
5. NUREG 1021, Rev 11, ES-401, PREPARING INITIAL SITE-SPECIFIC WRITTEN EXAMINATIONS, page 9, Section D.2.f, bullet 4, states significantly modify each question by changing at least one pertinent condition in the stem and at least one distractor. The intent or objective of the question does not necessarily have to be changed. Adding or deleting irrelevant information and making minor changes (e.g., the unit number, component train, or power level when it makes no difference) would not be considered a significant modification to the question.

This question was changed by changing the pertinent condition in the stem (i.e. how long the TCCP had already been installed) and changing the time listed in the answer and all distractors. This requires the student to recognize that they were in a 90-day time clock, know that the time begins at installation not discovery, and then calculate the time remaining until the 50.59 must be completed. NRC: Updated question is SAT.

Reference Provided; 2X2 NRC: SRO Only Criteria needs to be explained in the pedigree. Note that DEOP 400-5, FAILURE TO SCRAM, is the given selected procedure for all possible answers.

Response

SRO Justification: Per ES-401, Attachment 2, section II.E, 10CFR55.43(b)(5) includes the selection of a procedure or section of a procedure to mitigate or recover or which to proceed. In the associated question, the decision NOT to isolate the MSIVs that failed to close would be an SRO level decision. Normally, for a failure 80 H 3  ? N E of an isolation valve to close, the correct action would be to isolate the valves that failed to function. The SRO must recognize that the need to protect containment overrides the need to close an MSIV that failed to close spuriously, and which has no indication of a valid Group 1 isolation signal. In this case containment is threatened and the guidance in DEOP 0400-05 for bypassing the MSIV low level and Off Gas High Rad isolations would be the correct choice in order to maintain the condenser as a heat rejection path outside of containment. Additionally, at Dresden the SRO makes the decision whether to terminate and prevent to -35 inches or to TAF. This was added to the pedigree. (Question ID 27834)

GENERAL COMMENT

S:

1. Bank questions are indicated by B; Modified are indicated by M; New questions are indicated by N
2. There are (RO)/ (SRO) questions with references/attachments provided.
