ES-401 Record of Rejected K/As Form ES-401-4 ILT 20-1 (2021-301) NRC Exam Tier / Randomly Reason for Rejection Group Selected K/A 2 / 1 RO 300000 K5.13 The instrument air system has filters, but there is no credible failure mechanism that would allow creation of a question with three credible distractors. K5 has only 2 areas with importance factors at or above 2.5.

Replacement K/A: 300000 K5.01 2 / 1 RO 261000 A2.14 The SBGT system does not have any interlocks, alarms, procedures or features that address high pressure conditions.

Could not write question with three credible distractors.

Replacement K/A: 261000 A2.06 2 / 1 RO 263000 G2.1.19 The DC electrical system limited inputs in the plant computer (R-viewer system). There is not enough diversity in this system to allow writing question with three credible distractors. Observation of this K/A is better suited for a scenario or a JPM.

Replacement K/A: 263000 G.2.1.30 2 / 2 RO 290002 K6.17 The TIP system, although it does go through the reactor vessel, has no credible loss or failure mechanisms that would impact the Reactor Vessel Internals. The new K/A was more operationally valid and has not been sampled.

Replacement K/A: 288000 K6.02 1 / 1 RO 600000 A2.06 Direct overlap with 290003 K5.01.

Replacement K/A: 600000 A2.05 We have been unable to create a question with credible distractors 2 / 1 RO 262001.A4.02 that is a high enough LOD but is discriminating. This question was used on the 19-1 NRC exam, and all operators failed it. Attempts to revise the question have failed to create a more discriminating version.
