AMPR-XI.M10, BAC AMP Eval Rpt, 4.1 Scope of Prog (p11/60) The ONS program manages the aging effects of LoM due to potential BAC by including all susceptible components and materials such as carbon steel or low alloy steel and copper structure or component (e.g. piping, hangers, valves, equipment fasteners, tubing floors, other equipment), including electrical Clarify: what are tubing floors?

2 AMPR-XI.M10, BAC AMP Eval

Rpt, 4.5 Monitoring and Trend (p19/60) ONS program maintains a list of all inactive and active borated water leaks, documentation of excessive BA deposits, discoloration caused by corrosion, and affected targets. Information on all leaks is captured in a leakage tracking database (Fluid Leak Management database). [ The document references AD-MN-ALL-0006 Fluid Leak Mgment]
1) A spreadsheet was previously posted to the portal (U2 BACC Entries 12-13-2019 to 8 2021). Earliest entry is 2/14/2020, so is 12/13/2019 correct? Is the posted spreadsheet the list referenced above?

If the posted sheet is for the list referenced above, can comparable spreadsheets be provided for U1 & U3?

2) How does BAC list interface with the Fluid Leak Management Database? Are they independent lists or is the BAC a subset of the FLM?

3a) I would like someone to discuss the document posted on

the portal to help me understand what it contains.

3b) Bottom entry says oil leak (work request 20166116). If this is a BAC issue, please explain it.

3c) For the AR listed as 3236530, is this number correct?

This number is not in the OpEx report.

3d) Why is AR 2345479 not in the OpEx report? (beyond cut off date?)

3e) AR2331239 (5/20/2020) U2 pump disch flange boron accumulation, previously IDed as inactive, now has what is considered an active leak. Is this issue on the spreadsheet? (The spreadsheet has a 4/28/2020 item for this component, but it says Cancel 3f) AR2319477 (3/5/2020) 2-GWD-VA-0153. Request Engineering eval due to excessive leakage. Should this be included on the list referenced above?

3 Program Effectiveness Audit (OE8)

The staff notes that based on discussion in Objective 3 for Corrective Actions (NCR-2326562 for 2LP-VA-203) AR reviews are part of the audit.

The monitoring and trending program element (discussed above) says the program Did the program effectiveness audit look at the list being maintained by the program to validate that all boric acid leaks were being captured in order to allow monitoring and trending?

If not done as part of the program effectiveness review, is

maintains a list of all borated water leaks.

the list validated through some other periodic process?
