SUMMARY

S/G 1 During the cycle 3 inspection (September 1985), defects were found in four row 1 U-bend tubes, and the conservative decision was made to preventively l

plug all row 1 tubes. During the cycle 3A inspection (March 1987), all row 1 tubes were deplugged, U-bend heat treated, and eddy current examined in the U-bend with the then state-of-the-art Combustion Engineering (CE) row 1 and 2 segmented bobbin coil probe. As a result of these examinations, five row 1 tubes were plugged with CE iron band plugs. During the cycle 3B inspection (July 1988), the CE 1ron band plugs were removed and replaced with Westinghouse plugs. Row 1 U-bends were examined with an enhanced Zetec MRPC, and as a result, four additional row 1 tubes were plugged with Westinghouse mechanical plugs. One tube (RIO C44) was preventively plugged and stabilized due to TVA's response to NRC Bulletin 88-02. A total of 10 tubes are presently plugged (refer to Table 7).

S/G 2 During the cycle 3 inspection (September 1985), all 94 row 1 tubes were preventively plugged. During the cycle 3A inspection (March 1987), all row 1 tubes were deplugged, U-bend heat treated, and eddy current examined with the then state-of-the-art CE segmented bobbin coil probe. As a result of the inspection, three tubes were plugged with CE iron band plugs. During the cycle 3B inspection (July 1988), the CE iron band plugs were removed and replaced with Westinghouse mechanical plugs. Row 1 U-bends were examined with an enhanced Zetec MRPC probe and as a result, seven additional row 1 tubes were plugged. A totel of 10 tubes are presently plugged (refer to Table 7).

S/G 3 During the cycle 3 inspection (September 1985), all 94 row 1 tubes were preventively plugged. During the cycle 3A inspection (March 1987), all row 1 tubes were deplugged, U-bend heat treated, and eddy current tested with the then state-of-the-art CE segmented bobbin coil probe. As a result of the inspection, three tubes were plugged with CE iron band plugs. During the cycle 3B inspection (July 1988), the CE iron band plugs were removed and replaced with Westinghouse mechanical plugs. Row 1 U-bends were examined with an enhanced Zetec MRPC probe, and as a result, 21 additional row 1 tubes were plugged. A total of 24 tubes are presently plugged (refer to Table 7).
