VIRGINIA VPAP-1901 POWER REVISION 8 PAGE 5 OF 45 1.0 PURPOSE 1.1 This procedure establishes the requirements, responsibilities, and methodology to ensure personnel safety and accident prevention during the conduct of work activities. Additionally, it implements the policy which stipulates that safety is the primary consideration ofindividuals performing work at North Anna and Surry Power Station.

1.2 This procedure endorses the Nuclear Operations Industrial Safety & Health Accident Prevention Manual.

1.3 This procedure establishes the goal to achieve an accident-free and injury-free workplace at

  • North Anna and Surry Power Station. '

l.4 Where changes in work practices and/or conditions are diffemnt can assumptions made for this procedure, which are Safety / Loss Prevention and Health concerns, the Station Safety and Loss Prevention Department has the final decision. Changes or clarifications may be given through verbal or written instructions.

2.0 SCOPE 2.1 This procedure applies to Virginia Power employees, contractors, and vendors who perform work activities at North Anna Power Station and Surry Power Station.

2.2 Requirements for Industrial Respiratory Protection issues are contained within VPAP-1902, Industrial Respiratory Protection.

VIRGINIA VPAP-1901 POWER REVISION 8 PAGE 7 OF 45 4.0 DEFINITIONS 4,1 Accident An unintended event which results or has the potential to result in illness, injury, property damage, or environmental impairment.

4.2 Competent Person An individual approved by Superintendent of Maintenance who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to the employee. This individual has the authority to take-prompt corrective measures to eliminate them and has received the Environmental Projections .

Agency (EPA) approved training course in removal of gaskets containing asbestos.

4.3 Hazardous Atmosphere Any atmosphere that may expose employees to the risk of ceath, incapacitation, impairment of ability to self-rescue, injury, or acute illness from one or more of the following causes:

  • An oxygen deficient atmosphere containing less than 19.5% by volume or an oxygen enriched atmosphere containing more than 23.5% oxygen by volume.

A flammable gas, vapor, mist or airborne dust in excess of 10% of its lower flammable limit (LFL).

. An atmosphere having a concentration of any toxic substance above the permissible exposure limit (PEL see MSDS for material used). ,

i Any other atmospheric condition that is immediately dangerous to life or health (IDLH) 4.4 Immediately Dangerous to Life or Health (IDLH)

Any condition that poses an immediate or delayed threat to life or that would cause irreversible adverse health effects.  ;

4.5 Investigation A systematic examination, or detailed inquiry, of the factors contributing to an accident and the  !

4.6 Line Breaking The intentional opening of a pipe, line or duct that is or has been carrying flammable, corrosive, or toxic material, an inert gas, or any fluid at a volume, pressure or temperature capable of causing injury.

4.8 Report A written account of the findings of a completed accident investigation.

4.9 Significant Accident An accident involving, or having a high probability of involving employee or contractor disability, especially lost time or hospitalization, major property damage, unfavorable j publicity, adverse employee-relations, or environmental impairment, where notification of I outside agencies is required. )

5.0 RESPONSIBILITIES 5.1 Vice President - Nuclear Operations l The Vice President is responsible for the overall Industrial Safety and Health Program for the Nuclear Power Stations.

5.2 Site Vice President The Site Vice President is responsible for the implementation of the Industrial Safety and l Health Program at the Nuclear Power Station.

5.3 Manager - Station Safety and Licensing The Manager - Station Safety and Licensing is responsible for the overall development, coordination, and implementation of Industrial Safety and Health practices at the Station 5.4 Supervisor Station Safety and Loss Prevention The Supervisor Station Safety and Loss Prevention is res;ionsible for implementation of the Industrial Safety and Health practices at the Station. Specific responsibilities include the l

following:

5.4.1 Evaluating the effectiveness of Industrial Safety and Health practices at the Station. j 5.4.2 Ensuring the establishment of a Safety Committee.

5.4.3 Advising the Station Safety Committee on safety issues.

l 5.4.4 Ensuring that Station personnel are aware of Industrial Safety and Health issues.  !

5.5 ' Superintendent Nuclear Training l

The Superintendent Nuclear Training is responsible for the following:

5.5.1 ' Providing general awareness of Station Safety issues during Nuclear Employee Training (NET) and Nuclear Employee Retraining (NER) classes. -

5.5.2 Providing and coordinating First Aid Team training.

5.6 Superintendent of Outage and Planning The Superintendent of Outage and Planning is responsible for identifying industrial safety related concerns during preparation of a Work Order Package.

1 5.7 - Station Supervisors l

Station Supervisors are responsible for the following: )

5.7.1 Maintaining a safe work environment free of recognized hazards for individuals working under their supervision.

5.7.2 Ensuring that individuals working under their supervision comply with Industrial Safety and Health requirements.

5.7.3 Accompanying individuals working under their supervision to the doctor / hospital resulting from a job related accident.

5.7.4 Ensuring that accidents are investigated prior'to the end of the shift or as soon as practical.

l 5.7.5 Preparing an Accident Report for all accidents and submitting it within 3 working days to the local Safety Department.

5.7.6 Ensuring that craft made tools and devices are approved prior to use. Approval is 4 I= obtained in accordance with MDAP-0028, Control of Fabricated and Modified Tools.

5.8 First Aid Team The First Aid Team is responsible for the following:

l 5.8.1 Providing First Aid to accident victims.

1 5.8.2 Notifying the control room of the nature and location ofIndustrial Safety and Health emergencies.

5.9 Station Personnel Station personnel are responsible for the following:

5.9.1 Maintaining a safe working environment. i 5.9.2 Remaining aware of the requirements of the Nuclear Operations Industrial Safety &

l Health Accident Prevention Manual.

5.9.3 Notifying their supervisor of any work being performed in an unsafe manner.

5.9.4 Providing initial First Aid to accident victims,if knowledgeable.

5.9.5 Notifying the control room of accidents and their location.

5.10 Station Nurse j The Station Nurse is responsible for the following:

.5.10.1 Publishing a monthly memorandum listing the names of employees due medical work-up/ physical.

5.10.2 Scheduling employees for medical examinations / tests by making appointments based on first call, first served basis.

5.10.3 Ensuring that each employee receives a baseline exam for the following:

l a. Audiogram

b. Visual testing

!- e. Pulmonary function (Respirator users only)

d. Complete blood count (Respirator users only)
e. Urine (Respirator users only) 5.10.4 Entering appropriate medical clearance dates into Nuclear Human Resource System (NHRS) for retrieval by Exposure Control and Training.

5.10.6 Maintaining all medical records of Station employees and forwarding a duplicate copy to Employee Health Services for filing.

5.10.7 Notifying individual employees of abnormal findings and forward a copy of the records to the employee's personal physician or other health care provider upon request.

5.10.8 Providing First Aid and medical care for all personnel injured while working on site. .

6.1 General Safety Requirements Safety Requirements shall be developed, implemented, and administered to ensure that personnel safety is the primary consideration during work activities at North Anna and Surry Power Station.

6.1.1 Safety Requirements shall consist of the following elements:

. Nuclear Operations Industrial Safety & Health Accident Prevention Manual

. Safety Committee .

. Safety Meetings

. Safety Training / Orientation

. Safety Observations 6.1.2 The Nuclear Operations Industrial Safety & Health Accident Prevention Manual provides requirements and regulations for the safety of employees. Virginia Power employees shall:

a. Receive individual copies of the Nuclear Operations Industrial Safety & Health Accident Prevention Manual.
b. Review the Manual to familiarize themselves with its content.
c. Abide by.the requirements when performing work activities that are contained within the Manual.

6.1.3 A Station Safety Committee shall be established to ensure that personnel safety receives constant management attention.

a. Membership of the Station Safety Committee shall consist of the following:

. A representative from each Station department

. A representative from eat h matrixed Station department (e.g., Training)

. An elected Chairman

. An elected Vice Chairman l . An elected Secretary I

b. The Station Safety Committee shall conduct committee meetings on a periodic basis to review Station safety issues. The following guidelines shall apply:
1. The minimum number of members to establish a quorum shall be four.
2. Unscheduled meetings may be convened at any time by the Chairman, or in his/

her absence, the Vice Chairman.

3. The Committee shall provide recommendations to the Site Vice President on-l safety matters that involve excessive time, effort, or cost.
4. The Secretary shall maintain meeting minutes to document the following:

. Summary of actions taken

. Summary of reviews, evaluations, and recommendations

5. The Secretary shall present the meeting minutes to the Committee at the following meeting, and with the approval of the Committee, the minutes shall be signed by the Chairman.
6. The Secretary shall distribute the meeting minutes as a minimum to the following individuals:

. Site Vice President

. Manager Station Safety and Licensing

. Supervisor Station Safety and Loss Prevention

c. The Station Safety Committee shall perform the following for' safety hazards that present a serious or imminent threat to Station personnel
1. Immediately notify the Site Vice President of the condition. j
2. Assist Station management in notifying employees of the hazard.

3. Assist Station managementin eliminating the hazard,
d. The Station Safety Committee shall maintain records of Station Safety Committee meeting minutes.

6.1.4 Station departments shall conduct safety meetings to ensure that department personnel are aware of safety issues. Department safety meetings may consist of the following elements:

a. Reviews of recent Station accidents and discussion of preventive measures to prevent recurrence.
b. Recognition of department personnel who have contributed to the Station Safety Program.
c. Presentations on First Aid techniques by qualified individuals.
d. Safety precaution discussions during pre-job briefings.

6.1.5 The Superintendent Nuclear Training shall provide general awareness of Station ,

Safety issues during Nuclear Employee Training (NET) and Nuclear Employee Retraining (NER) classes.

6.1.6 The Supervisor Station Safety and Loss Prevention may recommend safety topics for NET and NER to the Superintendent Nuclear Training. The Superintendent Nuclear Training shall determine whether these topics are appropriate for NET and NER classes.

6.1.7 The Supervisor Station Safety and Loss Prevention shall conduct periodic safety observations at the Station.

l 6.2 Industrial Safety Inspections / Observations The Supervisor Station Safety and Loss Prevention shall ensure that Industrial Safety inspections / Observations are conducted on an as required basis. Industrial Safety Inspections /

Observations shall include observations of jobs for unsafe acts on a random basis and appropriate actions taken when safety deficiencies are discovered.

6.3 Reporting Safety llazards 6.3.1 Station personnel shall immediately report safety hazards to his/her supervisor, if possible, the individual shall correct the problem. Deviation Reports (DRs) should be submitted,if warranted, in accordance with VPAP-1501, Deviation Reports.

NOTE: Based upon the imminent potential for further hazards to Station personnel, the Supervisor Station Safety and Loss Prevention may initiate immediate corrective action to eliminate the hazard.

6.3.2 The supervisor responsible for the incident shall perform the following:

a. Ensure that the condition is corrected, including actions to prevent recurrence.

b. Report the corrective action to the Supervisor Station Safety and Loss Preventic a f and the Site Vice President. l ]

6.4 Accident Reporting Station Personnel involved in accidents, incidents, or damage to company property shall immediately report the event to their immediate supervisor, or any titled supervisor. Failure to report immediately will be considered a willful violation. The following requirements apply:

NOTE: Personal injury accidents which require offsite medical attention or driver controllable vehicle accidents may require For Cause Testing. Refer to VPAP-0105, Fitness For Duty Program, for guidance.

l 6.4.1 The supervisor shall perform the following:

a. Provide for immediate medical care of employees.
b. Investigate accident and submit preliminary information to department head before the end of the shift or as soon as practical.

c. Immediately notify Station Safety and Loss Prevention of serious accidents requiring doctors care in which company employees or non-employees are involved f

or where property damage occurs.

d. The immediate supervisor will accompany the injured employee on initial doctor and/or hospital visits resulting from company related accidents;
e. Prepare an accident report for all accidents using the following forms:
1. Employee injury Report (form 721038) required for all injuries to Virginia Power employees.

2. Choice of Doctor Acknowledgment (form 722708) required for employees I seeing a doctor. 1
3. Medical Care Authorization (form 720258) required to be completed for each doctor visit.

4. Miscellaneous Accident Report (form 731055) required for damage to Virginia Power property, vehicle accidents, and injuries to contractors /public.

5. All contractor accidents involving personal injury or company vehicles shall be reported immediately on the Miscellaneous Accident Report (form 731055).

The fonn shall be sent to Station Safety and Loss Prevention,

f. Notify the Supervisor Station Safety and Loss Prevention of accidnts that involve the following:

. Injuries requiring a doctor's attention i 1

. Company property damage Vehicle accident -

Report all accidents / incidents on the S AFE Hot Line by the end of the work shift. >

g. Forward completed Employee Injury Report / Miscellaneous Accident Report forms within three (3) working days to the Supervisor Station Safety and Loss Prevention.

6.4.2 The Supervisor Station Safety and Loss Prevention shall perform the following:

a. Determine if the accident is significant and requires notification to the Site Vice l j President. Significant accidents are as follows:  ;

1 NOTE: VPAP-2802, Notifications and Reports, provides additional requirements for accident reporting.

1. Fatalities.
2. Serious injury (i.e., Individual hospitalized or taken to hospital for decontamination).
3. An employee injures a non-employee.
4. Extensive damage to company or public property.
5. Serious incident, reportable under 10 CFR 50.72, immediate Notification Requirements for Operating Nuclear Power Reactors (i.e., Serious personnel injury, major daniage to Station equipment, or other significant occurrences).
6. Near miss accidents where potential for serious injury or damage was present.
b. Perform a detailed investigation of the accident and initiate corrective action to

> prevent recurrence.

c. Obtain a statement from the individualinvolved in the accident.

6.4.3 The Site Vice President shall notify the Vice President Nuclear Operations of any l

significant accidents.

6.5 Confined Space Entry See VPAP-1904, Confined Space Entry Program l

6.6 Asbestos Control Employee contact with asbestos containing materials should be kept to a minimum. Most asbestos removal jobs should be done by a qualified, licensed asbestos abatement contractor.

Small scale, specialized asbestos jo'os may be done by employees who have had proper, approved training.

NOTE: The following Steps identifies requirements for the removal or dismantling of equipmentjoined together by a gasket containing asbestos material.

6.6.1 The Superintendent of Maintenance is responsible for identifying all station personnel that may be involved in the dismantling of equipment joined together by gasket material classified as ACM (Asbestos Containing Material) or PACM (Presumed Asbestos Containing Material).

6.6.2 The Superintendent of Outage and Planning is responsible for identifying all tasks in which asbestos (ACM) or (PACM) gasket material may be encoun'tered.

6.6.3 A competent person is one who is capable of identifying existing asbestos hazards in the workplace and selecting the appropriate control strategy for asbestos exposure, who has the authority to take prompt corrective measures to eliminate them.

9 6.6.4 The components must be evaluated to determine if the gasket material used between connection points contain asbestos "ACM" all components that cannot be identified as non-asbestos must be treated as "PACM".

. Determine if the removal or dismantling of any component will cause a H!sturbance of the ACM or PACM gasket material

. The competent person will conduct an initial exposure assessment to ascertain expected exposures during the operation

. The competent person will supervise the operation and identify necessary steps to ,

prevent accidental exposure to the ACM or P/s :M gasket 6.6.6 Training Requirements:

6.7 First Aid Team A First Aid Team shall be established to ensure that emergency medical care is provided to injured /ill personnel. The First Aid Team shall be established in accordance with the following guidelines:

6.7.1 The Supervisor Station Safety and Loss Prevention shall establish and direct the First Aid Team. The primary mission of the First Aid Team is Emergency Medical Services (EMS).

c. Review medical procedures involving pre-nuspital care for the sick or injured.
d. Ensure that actions of the First Aid Team are in compliance with standards of the State of Virginia.

e.. Act as a liaison with local rescue squads.

6.7.4 The EMS Training Officer / Nuclear Training shall perform the following activities:

a. Document and maintain training records of EMS members,
b. Coordinate training and r: certification of EMS members with the following: -
1. Supervisor Station Safety and Loss Prevention.
2. Superintendent Nuclear Training,if required.

, . c. Conduct training exercises as directed by the Supervisor Station Safety and Loss Prevention.

6.7.5 First Aid team membera shall maintain one of the following minimum certifications: i I

. Medic First Aid / Emergency Vehicle Operators Certification (EVOC) '

. Emergency Medical Technician (EMT)

' 6.7.6 First Aid Team members must be EVOC trained to drive a Virginia Power ambulance.

' Any individual who is certified EVOC/ Medic First Aid may become a member of the First Aid Team upon approval of the Supervisor Station Safety and Loss Prevention.

6.8.1 Prior to initiating drilling, digging, or cutting, the supervisor responsible for the work shall ensure that Attachment 2, Drilling, Digging, and Cutting Permit, detailing the location of any embedded or buried electrical circuit or piping is completed.

a. The requesting department shall review the DDC Permit and perform the following, as appropriate:
1. Review drawings, when available.
2. Walkdown oflocation.

3. Discussions with Station personnel familiar with the work location.
4. Contact appropriate individuals to locate or move any utilities installed by organizations other than Virginia Power.
b. If scanning is required, contact the NSS Electrical supervisor to have an individual qualified to operate the scanning equipment perform the scanning and sign and date the DDC Permit in Block 9.
c. The individual performing the review shall complete the Engineering Instructions /

Special Precautions section of the DDC Permit, as appropriate.

NOTE: If positive location of embedded or buried electrical circuits cannot be obtained, ensure that rubber boots and Class Il rubber gloves are checked yes in the Engineering Instructions /Special Precautions section of the DDC Permit. '

d. Upon completion of the review, the individual performing the review shall sign and date Block 10 of the DDC Permit.
e. The requestor shall sign and date Block 11 of the DDC Permit indicating that any Special Precautions are understood.
f. An authoria.ed individual from the requestir.g organization shall perform one of the following:
1. Approve the CDC Permit by ass:gning a Permit number, expiration date, and signing Block 15 and providing to the requestor. l
2. Deny authorization, by marking " Void"in Block 15 and return to the requestor with an explanation of the denial.

g. The requesting organiz stion shall maintain the approved DDC Permits.

l 6.8.2 The supervisor responsible for the work shall perform the following: i

a. Post the DDC Permit in the proximity of the drilling, digging, or cutting.
b. Ensure that rubber boots and Class Il rubber gloves are wom by workers when positive location of embedded or buried electrical circuits cannot be obtained. l
c. Notify the individual who approved the DDC permit ifinterferences are encountered during the work. .
d. Upon completion of the work, notify the individual who approved the DDC permit -

if any underground utilities are discovered during the work. l

e. Upon completion of the work, ensure that block 16 of the DDC Permit is signed and

. returned to the requesting organization for a minimum of one year storage.

i 6.9 Portable Electrical Equipment 6.9.1 The following controls shall apply to equipment classified under an Assured Equipment Ground Program (color code,6.9.6, Table 1). They do not apply where a Ground Fault Circuit Interrupter (GFCI) Program is utilized.

6.9.2 GFCIs should be inspected annually by the Materials Management organization.

Written documentation to support the inspection shall be maintained on file by the Store Room.

NOTE: This subsection does not apply to portable electrical equipment with a two wire cord (e.g., radios, appliances. clocks, vacuum cleaners) or areas that are part of the permanent wiring of the building. Permanent electrical equipment with a grounding conductor (e.g., computers, printers, microwave ovens) may be inspected periodically at the discretion of the Supervisor Station Safety and Loss Prevention. Construction sites and wet / damp locations require the use of GFCI or an approved Ground Fault Assurance Program.

6.9.4 The Nuclear Materials Department is not required to provide an inventory to the Superintendent of Maintenance.

6.9.5 Station Personnel shall perform the following:

a. Visually inspect portable electrical equipment for external damage.
b. Notify their supervisor of portable electrical equipment with visual external damage.
c. Disconnect portable electrical equipment with visual external damage.
d. Ensure that portable electrical equipment has the current inspection code.

h 6.9.6 Station Supervisors shall perform the following:

a. Provide the Superintendent Maintenance with an inventory of portable electrical equipment on an annual basis,
b. Advise the Superintendent Maintenance of portable electrical equipment with visual external damage.

6.9.7 The Superintendent Maintenance and the Superintendent Station Materials shall ensure that all three wire or double insulated portable electrical tools and cord sets are inspemed and tested on a scheduled basis. Inspections and tests sliall be conducted as follows:

a. Prior to initial use.

, b. Prior to retum to service after repairs.

c. Prior to use after the equipment has been subjected to potential damage.
d. Every 3 months for portable electrical tools and cord sets, documentation is required as follows:
1. Portable electrical tools and cord sets s' 'l be identified with a unique number to ensure accurate recordkeeping. l

discretion of the Supervisor Station Safety and Loss Prevention.

6.9.8 The Superintendent Maintenance and the Superintendent Station Materials shall verify that portable electrical equipment has been inspected and tested through the use of color coded marking tape. Color coding shall be indicated as follows:

a. The Station Safety Department shall conduct periodic audio surveys and inspection of work areas which may require hearing protection to assess noise exposure and recommend feasible engineering and/or administrative controls.
b. Station personnel shall use ear plugs, ear muffs, or other hearing protection devices while in a Hearing Conservation area.
c. The Station Nurse shall conduct audiometric tests for all Virginia Power employees assigned to the Station.

2. Issue permanent hearing protection devices to personnel with an approved Stores Material Requisition.
3. Issue disposable hearing protection devices to personnel upon request.

a. Station personnel shall request Safety Glasses from Station Materials

c. Non-prescription eyewear shall be replaced on an as needed basis.
d. If an employee requires prescription safety glasses, the employee shall obtain an . ,

eye exam and prescription at the employee's expense. Upon completion of the eye ,

exam, the employee shall be fitted for a pair of prescription safety glasses by an optometrist.

1. Virginia Power will purchase prescription glasses on behalf of the employee, one pair every three years. Replacement glasses outside of this three year interval will be authorized only if the employee's prescription changes, or if damaged due tojob related work activities. Replacement glasses damaged due to job related work activities will require approval from the employee's supervisor.
2. Virginia Power will only offer one style of glasses at company expense. If a different style is desired, the employee will pay the co-payment above the company's authorized amount.
3. Sunglasses /photogreys will be purchased by Virginia Power only when specified on the prescription as " required due to medical reasons".
e. Contact lenses are authorized to be worn subject to the following:
1. When in an eye protection area, ANSI approved safety glasses shall be worn with contact lenses, except when utilizing full face respiratory protection.
2. When in an area of high dust hazard or where there is a possibility of a chemical spill that could affect the eyes, goggles shall be worn with contact lenses.
f. Personnel shall not use dark glasses (sun glasses), whether they are ANSI approved or nat, inside buildings

6.12 Contractor Accident / Incident Rate Performance Injury trends of contractor personnel shall be monitored by the Supervisor Station Safety and Loss Prevention. The following requirements apply:

6.12.1 An Accident / Incident Rate goal shall be established by Station Safety to rnonitor contractor injury trends. The following requirements apply:

a. Utilize Bureau of Labor Statistics Standard, Recordkeeping Guidelines for Occupational Injuries and flinesses to calculate the Accident / Incident Rate.
b. Collect data on accidents in the Virginia Power Accident Reporting Information System (ARIS).

6.12.2 The Station Safety Department shall perform the following:

a. Notify contractors of the established Accident / Incident Rate goal prior to initiation of work activities.
b. Inform contractors of their performance in meeting the Accident / Incident Rate goal at the first of each month,
c. Require contractors to report accidents on the Accident / Incident Report if offsite medical care is required.
d. Require contractors tc submit on a monthly basis the number of hours worked during the month to be entered into ARIS.
e. Contractor accidents / incidents are reported on ARIS to Corporate Safety as they occur, i

VIRGINIA VPAP-1901 POWER REVISION 8 PAGE 32 OF 45 6.13 Control of Compressed Gas Cylinders Control of compressed gas cylinders shall be performed in accordance with the requirements of the Nuclear Operations Industrial Safety & Health Accident Prevention Manual. The I following requirements apply:

NOTE: The requirements for compressed gas cylinders do not apply to Ponable Fire Extinguishers.

6.13.1 Compressed gas cylinders may be rolled on their bottom edge, but not dragged.

6.13.2 Compressed gas cylinders shall be stored as follows:

a. In a secured upright position with chains or metal straps.
b. Compressed gas cylinders may be stored in carts if the carts prevent the cylinders from dislodging in the event that the cart should tip over.
c. Compressed gas cylinders shall not be stored on roofs without the approval of the Superintendent Maintenance, Superintendent Site Services, or the Supervisor Station Safety and Loss Prevention.
d. Compressed gas cylinders, which are not permanently installed, shall not be stored in Safety Related Areas unless they are adequately restrained or they are evaluated for potential impact prior to placing them in the area.

6.13.3 Compressed gas cylinders shall not be dropped or allowed to strike each other.

6.13.4 Combustible materials shall not be stored adjacent to compressed gas cylinders.

6.13.5 Valve protection caps shall not be used to lift compressed gas cylinders and shall remain in place during transportation, handling, and storage.

6.13.6 Compressed gas cylinders shall not be used as rollers or suppons whether they are empty or full.

6.13.7 Compressed gas cylinders with flammable gases shall not be connected or disconnected frorr 3 gulators or lines in the presence of open flames, sparks, or operating electncal equipment.

6.13.8 Compressed gas cylinders shall not be transported in the horizontal position.

1. The Station Medical Department shall provide the individual with a copy of ANSI /ANS-3.4-1983 to forward to her physician. The individual shall provide the Station Medical Department with a written narrative from her physician as to when she will no longer meet ANSI /ANS-3.4-1983,5.4.6 with regard to the wearing of all types of respiratory equipment and 5.1-restricted mobility. At the time determined by the physician, the individual shall be administratively restricted from assuming Licensed Control Room Operator or Senior Operator responsibilities. This shall be considered a temporary condition and does not require notification to the NRC. The individual shall be assigned to such duties, if any, as are consistent with her physicians' description of her restrictions and capabilities.
2. If the individual declares her pregnancy for radiological purposes, she shall be j considered non-essential personnel (VPAP-2101, Radiation Protection Program) and administratively restricted,

6.14.3 Respiratory Clearance forIwn Employees

c. If the non-employee has qualifications or restrictions listed on the Non-Employee l Medical Clearance form or does not meet the Virginia Power Medical Standards required to wear a respirator, the Station Nurse shall forward the form to the Employee Health Services physician for additional review and disposition.

6.15 Site Orientation for Non Badged Personnel Non-Badged personnel shall receive site orientation training in accordance with VPAP-0201, Employee Check-In/ Check-Out. The site orientation training shall address j Industrial Safety, Loss Prevention, and the Site Emergency Plan. .

6.16 Safety Deficiency Report -

A Safety Deficiency Report, Attachment 4, may be used by Station personnel to document i unsafe conditions at the Station.

6.16.1 Upon completion of a Safety Deficiency Report, Station personnel shall forward the Report to the Supervisor Station Safety and Loss Prevention, j 6.16.2 The Supervisor Station Safety and Loss Prevention shall review the Safety Deficiency j

. Repon and if warranted, forward it to the Station Employee Safety Committee for j appropriate action.

6.17 Lead Paint Removal Guideline

. Lead Paint Removal Guidelines shall apply to all work involving lead based paints, including, but not restricted to the following hazardous paint removal processes:

. Abrasive blasting

  • Welding

. Burning

  • Cutting

. Braising Grinding i

CAUTION: Lead is a toxic substance. Over exposure to lead by breathing or ingestion is harmful or l fatalif taken in large doses.

J 6.17.1 The collection of paint samples for bulk analysis, when done according to the Lead Paint Removal Guidelines, does not result in airborne exposure to lead. Therefore respirators and other protective equipment are not required when taking these samples.

6.17.2 Individuals working on lead paint removal tasks must be aware of safety precautions i and the requirements of the Corporate Lead Paint Removal Guidelines.

6.17.3 The Virginia Power Supervisor in charge of Virginia Power personnel or contractors who are to perform lead paint removal tasks, has the overall responsibility for:

a. Ensuring compliance with the Lead Paint Removal Guidelines,
b. Ensuring prior to the job startup that a compliance program is written and implemented as required by the Lead Paint Removal Guideline NOTE: Copies of the Lead Paint Removal Guideline are available from the Station Safety and Loss Prevention Department.  ;

6.17.4 Supervisors of personnel involved in lead base paint removal should have a copy of the Lead Paint Removal Guideline on hand and be familiar with the contents.

6.18 Silver Braze Containing Cadmium If approved procedures and material exist that address the use of cadmium free silver braze, silver braze containing cadmium shall not be used for brazing or any other s!milar operation.

In cases where cadmium bearing silver braze products are required, employees (Welder and Helpers) involved shall wear a full face respirator with high efficiency filters and notify the Station Safety Department prior to starting work in order to ensure compliance with other requirements that may apply to that specific job.
