POWER REVISION 5 PAGE 5 OF 33 1.0 PURPOSE This procedure establishes the Crane and Hoist Program to ensure safe and reliable operation of Station cranes and hoisting equipment in accordance with applicable standards and regulations. [ Commitment 3.2.2]

2.0 SCOPE 2.1 This procedure applies to Station personnel, including contractors, who issue, operate, and maintain Station cranes and hoists.

2.2 This procedure applies to all activities related to operation, inspection, testing, and maintenance of permanently installed Station cranes, hoists, and their associated equipment.

2.3 This procedure provides qualification and requalification requirements for Station crane operators (Virginia Power Personnel), with the exception of the Manipulator Crane, the Fuel Handling Bridge Crane, and the Remote Operated Fuel Building Cask Crane which are covered l in the Operator Development Program.

2.4 The issue of mobile cranes and mobile crane maintenance, testing, and inspection are delineated in VPAP-0202, Station Vehicle Control. However, the qualification and requalification of Station (Virginia Power personnel) mobile crane operators as delineated in this procedure is applicable.

2.5 Qualification and requalification requirements of Contractor crane operators is the responsibility of the Contracting Organization and is not controlled by the procedure.

- 3.2.3 QA Audit C95-08-03NS 4.0 DEFINITIONS 4.1 Crane A machine for lifting and lowering a load and moving it horizontally, with the hoisting mechanism an integral part of the machine.

4.1.1 Cab Operated Crane A crane controlled by an operator in a cab attached to the bridge or trolley.

4.1.2 Floor Operated Crane A crane which is controlled by a means suspended from the crane [and operated by an operator on the floor or an independent platform (manual or power operated).

4.2 Crane / Hoist Usage Log A document maintained by Nuclear Materials - Tool Control that is used to record crane / hoist usage. The completed Crane / Hoist Usage Log (Attachment 2) shall be returned to Tool t

The completed Crane / Hoist Operator's Checklist (Attachment 5) shall be returned to Tool Control )

i 4.4 Critical Lift l A lift involving equipment or materials which could result in equipment damages in excess of

$500,000, a forced outage of longer than eight (8) weeks, or could cause an uncontrolled  ;

release of contamination and/or radiation. This defm' ition is in keeping with the intent of Nuclear Mutual Limited.

EXAMPLE: Lifting heavy equipment over the turbine and/or any associated equipment under normal operations would be considered a critical lift.

4.5 Hoist A machinery unit that is used for lifting or lowering a freely suspended (unguided) load.

5.0 RESPONSIBILITIES 5.1 Department Heads Depanment Heads are responsible for:

5.1.1 Ensuring crane operators are qualified. This includes:

l

a. Ensuring personnel are trained and qualified in accordance with the requirements of this procedure before operating any crane independently.
b. Approving lists of Job Performance Measure (JPM) trainers and evaluators.

5.1.2 Ensuring appropriate procedures are written for lifting operations, as necessary.

5.1.3 Monitoring crane operations periodically to ensure that safety and good handling i practices are followed.

5.2 Superintendent - Maintenance The Superintendent - Maintenance is responsible for:

l 5.2.1 Maintenance, inspection, and testing of all permanently installed cranes at the Station.

5.3 Superintendent - Nuclear Training The Superintendent - Nuclear Training is responsible for:

5.3.1 Conducting On-the-Job Training (OJT/OJE) workshops for crane trainers / evaluators.

5.3.2 Developing and maintaining a currentlist of qualified trainers and evaluators for crane tasks performed by Station personnel.

5.3.3 De veloping and maintaining a current list of all qualified crane operators, including contractors.

5.3.4 Providing the Station Nurse a copy of the current list of qualified crane operators.

5.3.5 Approving requests for and coordinating training for Station personnel on crane operation and/or rigging.

5.3.6 Providing the Materials Process Team Leader and applicable department heads with a l copy of the current list of qualified crane operators.

5.4 Crane Program Coordinator The Crane Program Coordinator shall be designated by the Superintendent - Maintenance and shall be responsible for:

5.4.1 Maintaining a current list, by mark number, of the equipment covered by the Crane and Hoist Program.

5.4.2 Reviewing the results of annual code compliance inspections with the inspector to determine what corrective actions, if any, are required as a result of the inspector's findings.

5.4.3 Maintaining a readily accessible file of compliance inspection results for each crane, l

including records of all deficiencies noted and of all hook dimensional checks performed.

5.4.4 Assessing the effectiveness of the Crane and Hoist Program and recommending corrective actions as necessary.

VIRGINIA VPAP-0810 POWER REVISION 5 PAGE 9 OF 33 5.5 Supervisors Supervisors of any job involving the operation or maintenance of cranes or hoists are responsible for:

5.5.1 Authorizing crane operators, after evaluation, on applicable Job Performance Measures (JPMs).

5.5.2 Ensuring personnel operating Station cranes and hoists are qualified in accordance with the requirements of this procedure.

5.5.3 Ensuring that a pre-lift briefing is conducted prior to a lift.

5.5.4 Ensuring personnel who are being trained are under direct supervision of a qualified crane operator or vendor for training purposes.

5.5.5 Monitoring lifting operations.

5.5.6 Notifying the Station Nurse of physical or emotional problems that may jeopardize safe operation of cranes.

5.6 Crane Operators Crane Operators are responsible for:

5.6.1 Performing an inspection of cranes and hoists prior to making any lift.

5.6.2 Performing lifting operations in a safe manner in accordance with applicable standards referenced in 3.1.2 and 3.1.4.

5.6.3 Notifying their immediate supervisor of physical or emotional problems that may jeopardize safe operation of cranes.

5.7 Signal Man A Signal Man is responsible for:

5.7.1 Maintaining good communications with the crane operator. This includes verifying crane signals to be used during the lifting operation are understood with the crane operator prior to performing the lift.

5.7.2 Ensuring radio or powered headsets required for communications during lifting operation are operable, prior to performance of the lift,if applicable.

PAGE 10 OF 33 5.7.3 Reviewing all aspects of the lift operation and walking down the load path prior to performing the lift.

5.7.4 Stopping lifting operations when any unsafe conditions exist which could cause equipment damage or endanger personnel.

5.8 Craftsmen Craftsmen involved in lifting operations are responsible for:

5.8.1 Performing lifting operations in accordance with approved procedures,if applicable.

5.8.2 Verifying the integrity of rigging, cribbing, and lifting inserts for blocks prior to use.

5.8.3 Following safe work practices for lift operations in accordance with the Nuclear

' Operations Industrial Safety and Health Accident Prevention Manual.

5.8.4 Verify current quarterly inspection of all rigging hardware and notifying tool control personnel to obtain an inspection prior to use when needed.

5.9 Station Nurse The Station Nurse is responsible for:

5.9.1 Reviewing physical records of Virginia Power crane operators to determine if operators meet the fitness requirements in accordance with applicable standards referenced in 3.1.2.

5.9.2 Notifying the crane operator's immediate supervisor and the Supervisor Nuclear Training EMI when a crane operator fails to meet the fitness requirements in accordance with applicable standards referenced in 5.9.1.

6.1.1 General

a. 'A list of cranes and hoists applicable to the Crane and Hoist Program shall be maintained by the Crane and Hoist Program Coordinator. )
b. A list of all qualified crane operators (including contractors) shall be developed and maintained by the Nuclear Training Department.
c. A list of qualified trainers and evaluators for crane tasks performed by Station personnel shall be developed and maintained by the Nuclear Training Department in accordance with VPAP-2302, On-The-Job Training and Evaluation.

'l d.- Qualification of contractor crane operators shall be documented on Contractor Crane Operator Qualification Sheet (Attachment 1).

1. The Contractor Crane Operator Qualification Sheet shall be completed by the contractor organization and must be approved by the Virginia Power department head to whom the contractor reports. j

~2. The original completed form shall be forwarded to the Supervisor Nuclear Technical Training and copies forwarded to the individual, contractor organization, and Materials Process Team Leader.

3. Contractor crane operators shall meet the applicable physical requirements for crane operators delineated in:

. ANSI B30.2-1976 and ANSI B30.17, Overhead and Gantry Cranes

. ANSI B30.11-1973, Monorail Systems and Underhung Cranes

. ANSI B30.5a-1984, Mobile and Locomotive Cranes

'4. Contractor crane operators shall arrive on site physically qualified to meet the  !

l requirements in 6.1.1.d.3. This shall be verified by the Virginia Power I department head.

e. With the exception of floor - operated cranes / monorails, all crane operators shall meet the applicable physical requirements for crane operators as delineated in ANSI L

B30.2.0, ANSI B30.5a, ANSI B30.11and B30.17. The physical requirements are:

. At least 20/30 vision in one eye and 20/50 in the other, with or without corrective lenses

. Ability to distinguish colors if color differential is required for operation -

  • Hearing, with or without hearing aid, adequate for specific operation (IAW Employee Health Services Directives and Policies Manual for Virginia Power Crane Operators)[ Commitment 3.2.3]

. Absence of physical defects or emotional instability which could render the

- operator a hazard to himself or others, as determined by annual physical and medical history Not subject to seizures or loss of physical control based on medical history provided by the individual Normal depth perception, field of vision, reaction time, manual dexterity, and coordination No tendencies to dizziness or similar undesirable characteristics

f. The Station Nurse shall verify that Virginia Power crane operators meet the above

. physical requirements.

g. The Station Nurse shall indicate the Virginia Power crane operator has met physical requirements as follows:
1. The Station Nurse shall enter a pass or fail and the date into the NHRS certification area for crane operator medical certification. For initial certification a Medical Testing Form will be forwarded to the Supervisor Nuclear Training, EMI. For requalifying, the information may be retrieved by training from the NHRS certification area.

2. If the Station Nurse determines a Virginia Power crane operator fails to meet all of the applicable physical requirements during annual physicals or other I examinations, the following notifications shall be made: [ Commitment 3.2.3)

. The crane operator's immediate supervisor shall be notified that the individual no longer meets the physical requirements for crane operation

. The Supervisor of Training EMI shall be notified that the crane operator no longer meets the physical requirements for crane operation. This n. notification shall be in writing. A copy of the individuals's medical testing form indicating '

the crane operator no longer meets the physical requirements may be used for notification

h. The Supervisor Training EMI shall remove from the list of qualified crane operators the name of the crane operator who failed the physical requirements.
i. Maintenance on any crane or hoist shall be performed by individuals who are:

- Qualified on the applicable maintenance task, and i . Qualified crane operators or are accompanied by a qualified crane operator

j. Maintenance may also be performed by approved vendors.

6.1.2 . Qualification Requirements (This step does not apply to contractors)

In order for an individual to be qualified as a crane operator, the following three items must be satisfied:

. Complete knowledge testing of the crane to be operated (if applicable)

. Complete practical testing for the type crane to be operated

. Obtain supervisor signature on JPM form t

a. Knowledge Testing Crane operators, depending on crane type, must pass a written or oral knowledge examination in accordance with the documents listed below:

b. Practical Testing Crane operators are required to complete a practical test in the presence of a i qualified evaluator for each type of crane they operate.
1. Each crane operator shall pass a practical operating examination (Job Performance Measure [JPM])). The following JPMs may be obtained from the l Supervisor Nuclear Training EMI:

2. After successfully completing the applicable JPM, the crane operator and evaluator shall sign and date the JPM.

3. Supervisors shall sign the applicable JPM, after evaluation, authorizing the individual to operate the applicable crane. Supervisors do not have to be a qualified operator to sign JPM authorizing individuals to operate cranes.
4. The original JPM shall be forwarded to Supervisor Nuclear Training EMI.

6.1.3 Requalification of Crane Operators

a. All crane operators shall requalify on operation of cranes within the period listed below: (This step does not apply to Contractors)

b. Requalification shall be required anytime the required certification period has lapsed.
c. Requalification shall consist of verifying the current medical certification from NHRS and completing only the practical portion of the applicable JPM.
d. Supervisors and Nuclear Training Department personnel qualified to operate cranes shall not be required to requalify on operation of applicable cranes to maintain

, qua'lification to evaluate individuals on JPMs.

6.2 Crane / Hoist Usage Log and Crane / Hoist Operator's Checklist 6.2.1 The Crane / Hoist Usage Log (Attachment 2) shall be maintained by Tool Control for each permanently installed Station crane or hoist.

6.2.2 The Crane / Hoist Usage Log (Attachment 2) shall also be used to record usage for Maintenance Department personnel perfomiing preventive maintenance.

6.2.3 The Crane / Hoist Operator's Checklist (Attachment 5) shtill be maintained by Nuclear Materials - Toc! Control for each permanently installed Station crane or hoist and shall be completed each time a crane or hoist is operated.

VPAP-0810 VIRGINIA POWER REVISION 5 PAGE 16 OF 33 6.2.4 The operator shall perform the following visual inspections, as applicable, and at the specified frequencies. The results of these inspections, including any deficiencies shall be recorded on Crane / Hoist Operator's Checklist (Attachment 5).

6.3 Crane / Hoist Operating Permits

. 6.3.1 A Crane / Hoist Operating Permit (Attachment 3) shall be required for all crane / hoist l operations, including those required for inspection or maintenance with the exception of the:

a. Manipulator cranes
b. Fuel Handling Bridge Crane -
c. Trash Rake 6.3.2 Prior to crane / hoist operation, the crane operator shall reqtiest a Crane / Hoist Operating Permit (Attachment 3) from Tool Control.

NOTE: Copies of completed JPMs or Contractor Crane Operator Qualification Sheets may be used by tool control to issue crane / hoist operating permits to personnel between revisions to the list of qualified crane operators.

a. Issue crane / hoist operating permits only to personnel on the current list of qualified crane operators for the applicable crane.
b. Complete the applicable section of the Crane / Hoist Operating Permit (Attachment

Cranes and hoists require mechanical, electrical (if applicable), and compliance inspections prior to operation, unless all such inspections have t,een performed within the past 12 months,

d. If periodic inspection is found to be necessary, Tool Control shall not issue the permit other than for the sole purpose of performing such an inspection, unless a writtcn authorization is obtained from Maintenance Engineering tojustify use of the applicable crane.

6.3.4 The Crane / Hoist Operating Permit (Attachment 3) shall be valid for the period l requested by the operator, up to a maximum of 30 days. The expiration date shall not extend past the next required date of periodic inspection.

6.3.5 Each person operating the crane must record the following information on the pennit prior to operation:

2. A supervisor or Maintenance Engineer may void the permit by annotating the last entry in the Crane / Hoist Usage Log (Attachment 2) in Tool Control to l indicate that the permit has been lost and is declared void. The individual making the void entry shall sign and date the log entry. )
3. The supervisor or Maintenance Engineer should notify the cognizant supervisor, either verbally or by memo, that the permit has been voided. Ng.

1 6.3.8 Whenever possible, the Crane / Hoist Operating Permit (Attachment 3) shall be in the I operator's possession during crane operation. If this is not practical, such as for work inside the reactor containment, the permit may be attached to the applicable work order or working document. ,

6.3.9 Upon completion of the specified job or expiration of the permit, the permit shall be returned to Tool Control. If thejob is to continue past the expiration date of the permit, )

6.3.10 If a permit is not returned to Tool Control by its expiration date, Tool Control shall notify the cognizant supervisor of the person the permit was issued to, that all usage of the crane shall cease until a new permit is issued. The Supervisor shall ensure the

permit is returned or void the permit on the Crane / Hoist Usage Log (Attachment 2).

6.3.11 Upon receipt of the returned permit, Tool Control shall update the Crane / Hoist Usage Log (Attachment 2) for the specified piece of equipment with the dates of most recent i inspection.

6.4 Crane / Hoist Operation 6.4.1 Department heads shall ensure that appropriate procedures are developed for operation j of cranes / hoists. I 6.4.2 Pre-Lift Activities  ;

a. A pre-lift meeting shall be conducted by the supervisor or the Person In Charge {

(PIC) of the lift with affected personnel (e.g., operator, signalman, rigger). ,

b. A Pre-Lift Checklist from station procedures may be used for ensuring pre-lift activities are properly considered. Lifts less than 2,000 pounds at North Anna or less than 1,600 pounds at Surry do not normally require documentation unless the load will traverse over critical equipment in restricted load areas. The cognizant supervisor or PIC will determine if the checklist should be initiated. I
c. The path that the load will travel shall be identified and walked down prior to movement of any load.

d. The integrity of rigging, cribbing, lifting inserts for blocks shall be reviewed prior to use.
e. IE the lift is considered to be a critical lift AISIl the annual compliance inspection has not been performed within the last three months on the crane to be used for the lift, THEN request that a qualified crane inspector perform a Crane / Hoist Pre Critical Lift Inspection (Attachment 4) in conjunction with the Crane / Hoist Usage Log (Attachment 2).

6.4.3 Cranes / Hoists may only be operated by the following personnel:

a. Personnel who are qualified for operation of the crane / hoist that will be used.
b. Personnel who are being trained and are under the direct supervision of a qualified crane operator for training purposes.

6.4.4 The operator of the crane shall not engage in any practice which will divert his/her attention while actually engaged in operating the crane / hoist.

6.4.5 When physically or mentally unfit, an operator shall not engage in operation of the equipment.

6.4.6 Each operator shall be held accountable for the safe operation of the equipment being operated. Whenever there is any doubt to safety, the operator shall have the authority to stop and refuse to handle loads until safety has been assured.

6.4.7 For cranes equipped with warning devices, a warning signal shall be sounded before traveling, and continuously during travel, when such an operation may create a safety hazard to personnel.

6.4.8 Before leaving a crane unattended, the operator shall verify that all controllers are in the OFF position and open the main switch of the erane.

6.4.9 Before closing the main switch, the operator shall place all controllers in the OFF position.

6.4.10 All controls shall be tested by the operator before beginning a new shift. If any controls do not operate properly, they shall be adjusted or repaired before any operations begin.

If adjustments or repairs are necessary, then a Work Request should be initiated in accordance with VPAP-2002, Work Request and Work Order Tasks. l

VIRGINIA VPAP-0810 POWER REVISION 5 PAGE 21 OF 33 6.4.11 Clothing, tools, spare parts, fuses, and personal belongings should not be stored in the operating cab in such a manner as to interfere with access or operations.

6.4.12 The operator shall check the hoist brakes at least once each shift if a load approaching the rated load is to be handled. This shall be done by raising the load a short distance, verifying that the brakes engage automatically when the hoist is stopped, and verifying that slippage (if any)is minimal.

6.4.13 On rope hoists, the load shall not be lowered below a point where less than two full

wraps of rope remain on the drum unless a limit switch is provided. In this case, one l full wrap shall remain on each anchorage of the hoist drum. I 6.5 Safety Precautions 1

6.5.1 Signals to the reactor containment polar crane operator shall only be verbal. I transmitted by sound-powered phone (or other approved methods, as determined by Component Engineering) to the operator's headset by a person designated as the signal l man.

i 6.5.2 Upon loss of communication between the operator of an overhead crane and the i signalman, all controls will be placed in a safe position until reliable communications have been restored. i l

6.5.3 The operator shall respond to signals only from the appointed signal man. However, the operator shall obey a stop signal at all times, no matter who originates the signal.

6.5.4 Should the power supply be interrupted during operation of a crane or hoist, the operator shall immediately place all controllers in the STOP position. Prior to reuse of the crane / hoist, operating motions shall be checked for proper direction.

6.5.5 The operator of a crane shall not leave the controls while a load is suspended.

6.5.6 Sudden starts and stops of crane / hoist movement should be avoided.

6.5.7 When the weight of a load is not known or cannot be easily judged, the weight shall be determined by referring to the nameplate data or by use of a load cell.

6.5.8 Prior to placement of a load, the laydown area shall be reviewed for floor loading.

Deviation from approved laydown plans shall be reviewed and approved by engineering, prior to the lift.

PAGE 22 OF 33 6.5.9 A crane or hoist shall not be used to make any lifts while maintenance is being performed, unless the lift is part of the maintenance procedure (e.g., load test).

6.6 - Control of Heavy Loads Requirements delineated in VPAP-0809, NUREG-0612 Heavy Load Program, shall be adhered to for any load greater than 2,000 pounds at North Anna or greater than 1,600 pounds at Surry which is lifted over spent fuel, fuel in the reactor core, or equipment required for safe shutdown or continued decay heat removal.

6.7 Maintenance of Cranes and Hoists The Superintendent - Maintenance shall ensure that permanently installed Station cranes and hoists are evaluated for preventive maintenance requirements (e.g., maintenance, inspections, and testing)in accordance with VPAP-0803, Preventive Maintenance Program.

7.0 RECORDS Prior to transmittal to the Training Center or Station Records Management, the sender shall assure that:
