SWEC-SQN-59 ANI Inspection Report N229 - Material Handling I. INTRODUCTION During an inspection by American Nuclear Insurers (ANI) September 24-26, 1985 (reference 1), a visual surney of the unit 2 Turbine Building 200 ton overhead crane was performed. No major items of concern were noted; however, several questions requireo investigation and were included in the Stone & Webster Engineering Corporation (SWEC) systematic analysis as follows:

1. L99851212026-001 - Need fire extinguisher on crane bridge footwalk . level.
2. L99851212026-002 - Amount of cable left on hoist drum.

{ 3. L99851212026-OO3 - SQM-31, Sections remain uncompleted.

4. L99851212026-004 - Need Nondestructive test program for wedge

}4 sockets.

l Information on the background, the corrective actions taken, the verification methodology, the verification analysis, the completion status, and any pertinent references for the area of concern is included in this report.

II. VERIFICATION OF SWEC ISSUES i

A. L99851212026-OO1 - Fire extinguisher on crane bridge foot walk level.

1. Background - Although a portable fire extinguisher is provided in the crane cab, the question was raised by the ANI as to the physical capability of an individual to carry the unit from the cab up the ladder to the bridge footwalk i area. This area is where major electrical control cabinets for the crane are located. This should be investigated as to the need to provide portable fire extinguishers on the crane bridge footwalk level.
2. Corrective Action Taken - In their response (reference 2)

Sequoyah Nuclear Plant (SQN) disagreed with the ANI recommendation. SQN stated:

)

We do not require, or train, our crane operators to fight fires and would prefer that they not do so in the event of a fire at or above the bridge level on our overhead cranes. Since this function is assigned to our fire brigade, and they would have fire extinguishers available on their response, we do not feel the provision of additional fire extinguishers for these cranes is necessary.

3. Varificction Methodology. - Tha SWEC concern identified for Employees Concerns Task Group (ECTG) verification was stated as follows:

4. Verification analysis - The ECTG review determined that the ANI recommendation for portable extinguishers was not an official recommendation requiring action by SQN. The SQN position was communicated (reference 2) to the Power Financing Staff for forwarding to the American Nuclear Insurers. The item is considered by SQN to be closed.

5. Completion Status - Based on the fact that no corrective action

- is required, and that the SQN Safety Staff and Site Director have concurred on the Sequoyah position, this SWEC item can be

, closed.

B. L99851212026-002 Amount of cable left on hoist drum

1. Background - From the review of crane inspection records, a question developed as to the amount of wire rope that remains

- on the drum of the Unit 1 Turbine Building crane main hoist.

) Appendix A, " Crane Inspection Comments" of Procedure No.

! MI-9.4.1 indicates that two wraps remain on the crane drum when

=

the hook is at elevation 709' of the Turbine Building. The 4

lowest floor level that the crane hook can be lowered to is elevation 701'. The corrective action was to stencil the hook sleeve casing with a precautionary note to the effect of not to lower the hook any further than elevation 709' . No lower hoist limit switch is provided. The ANI inspector recommended that SQN investigate this matter in greater detail.

2. Corrective Actions Taken - The SQN response (reference 2) j concurred that the issue should be investigated and resolved.

! ECTG discussions with the SQN Modifications Supervisor

} indicated that a modification had been initiated to install a lower hoist limit switch in accordance with the ANI recommendation.

3. Vzrificetion Methodologyu Tha SWEC concern identified for ECTG verification was-stated as follows:

RIMS # ISSUE RIMS ITEM L99851212026 Concern with amount of RIMS-002 cable left on main & aux turbine crane hoist drums when the hooks are fully extended to lowest floor level.

ECTG reviewed the Sequoyah document control files for internal and external correspondence related to this issue. This review of the pertinent documentation, in addition to discussions with SQN industrial safety and mechanical maintenance personnel, formed the basis for this verification activity.

4. Verification analysis - ECTG verified that a provision for a lower hoist limit switch is included in Design Change Request No. 2264 titled " Safety Improvements on the Overhead Crane."

Engineering Change Notice No. 6721 incorporates the switch provision. The estimated completion date for the modification is July 1987.

5. Completion Status - Corrective action for this ANI concern has been initiated. Following completion of the planned modification, the SWEC issue can be closed.

C. L99851212026-OO3, SOM-31 Sections remain uncompleted.

1. Background

The ANI report stated:

Responsibilities for the inspection, testing, maintenance and operation of the various cranes, hoisting and rigging equipment utilized throughout the plant and site are outlined in Standard Practice SQM-31, " Inspection, Testing Equipment." Part 3.0 titled " Procedure" contains seven (7) sections which pertain to each particular type of equipment at the site, to the department assigned responsibility of the equipment and to the specific requirements for the equipment. Four (4) of the seven sections remain l uncompleted and indicate the section will be added at a later date. The omitted sections relate to mobile truck .

Also, Section 3.7 - Hooks would be applicable, and guidelines for this specific equipment device should be established. Revised sections of SQM-31 should be

submitted to ANI for review.
2. Corrective Action Taken - SQN's response stated:

IN SQM-31, we have indicated that these sections will be

{ completed at a later date. This should include l base-mounted drum hoists (Section 3.5), in addition to the i three sections (Sections 3.3, 3.4, and 3.7) included in ii ANI's concerns, since we currently use base-mounted hoists. All four of these sections are listed in SQM-31 as l Mechancial Maintenance's responsibility. Specific dates should be established for the completion of these sections and they should be tracked on either the Management Action

] documents indicated that the corrective actions for this 1 concern are in progress. The Sections 3.3, 3.4, 3.5, and 3.7 3 have been completed for inclusion in revision 4 of SQM-31.

5. Completion Status - Based on SQN's completion of the pertinent H SQM-31 sections and their scheduled issuance, no additional h actions are required on this issue. Following issuance of the j revised procedure, the SWEC concern can be closed.

D. L99851212026-004 Nondsstructiva test for Wedgw Sockets

1. Background -

The Wedge sockets used on Manitowoc cranes had defects (cracking) that were identified by the ANI. Nondestructive tests (NDT) were performed by the Power Service Shop on each wedge socket on the applicable cranes onsite, at SQN's request. The wedge sockets are used for securing certain ball type hooks. The ANI report suggests including the wedge sockets in the NDT program for the hooks.

2. Corrective Actions Taken - SQN's response (reference 2) stated:

The recommended action seems prudent. The investigation into this recommendation and implementation of action should be assigned to Mechanical Maintenance and tracked on either MATS or CATS.

3. Verification Methodology - The SWEC concern identified for ECTG verification was stated as follows:

4. Verification Analysis - ECTG verified through discussions with the SQN Mechanical Maintenance Supervisor that provisions for the inclusion of wedge sockets in the NDT program have been included in a draft, revision 5, to the Standard Practice Prccedure, SQM-31, " Inspection, Testing Equipment." The revision 5 is presently undergoing SQN quality assurance review.
5. Completion Status - Appropriate corrective action has been initiated. Following implementation of the planned actions, the SWEC issue can be closed.
