Report SWEC-SQN-35 NRC Report 50-327, 328/85-21 I. IKTRODUCTION During the time period June 3-7, 1985, the Nuclear Regulatory Commission (NRC) conducted a routine, unannounced inspection (reference 1) of design change activities reported as complete in SQNs annual operating report (reference 2). During this inspection, NRC identified two inspector followup items (327, 328/85-21-01 and 327 328/85-21-02) which were addressed as two issues in the Stone & Webster Engineering Corporation (SWEC) systematic analysis. These items are as follows:

i II. VERIFICATION OF SWEC ISSUES A. A02850722020-001, Plant Design Records Inadequate

1. Background 4

g During the review of the design change packages associated with is ECN No. 2404 and ECN No. L5550, certain documents were identified as missing from the packages obtained from the vault. These records were to provide objective evidence of activities affecting quality in connection with design changes reported as complete in the licensee annual operating report j (reference 2). At the request of the inspector, SQN was able to 1 provide some of the missing documentation. Additionally, SQN j identified for the inspector reference documents for cases where T the missing documents were not readily available. The inspector j also identified errors in the information concerning completed design changes contained in the annual operating report.

Until a reinspection of the design change program for design activities reported as complete in the annual operating report is performed, this was identified as Inspector Followup Item s (IFI) 327, 328/85-21-01.

2. Corrective Actions Taken A formal response from SQN was not required. ECTG interviewed SQN's document control unit supervisor who indicated that SQN has confidence in the present status of the design change packages. During recent months, page numbering of the packages' contents has been accomplished to help assure package completeness. A tightly controlled checkout system is kooping track of package locations.

An additional interview with SQN Compliance Section personnel indicated that some errors had been made inadvertently in the listings which appeared in the annual operating report, but that these errors were uniqae and would not occur again.

3. Verification Methodology The SWEC concern identified for ECTG verification was stated as follows:

ECTG reviewed the Sequoyah Compliance Licensing files for ,

internal and external correspondence related to this issue, and NRC status and closure files. This review of the pertinent documentation, in addition to interviews with SQN Document Control and Regulatory Compliance personnel, formed the basis for this verification activity.

4. Verification Analysis The ECTG review of the pertinent documents indicated that no additional corrective actions appear necessary for IFI 327, 328/85-21-02. At the time of this ECTG verification, the NRC IFI remained open. However, SQN personnel believe the item is ready for NRC closure.
5. Completion Status Corrective actions for this issue are considered complete by SQN. Following a reinspection by NRC and closure of the IFI this SWEC issue may be closed.

B. A02850722020-002, 0:vslopment of St*4ff Guidrnco for preparStion of USQD Form

1. Background SQN identified to the NRC inspector a need to revise SQN Standard Practice SQA 119, Unreviewed Safety Question Determination (USQD) to more adequately provide guidance to plant personnel in completing the USQD form. The inspector reviewed the format of the present form with SQN management and l discussed the need for similar guidance in the preparation of l

the Unreviewed Environmental Question Determination (UEQD) and the Environmental Qualification Protection Requirements. Until completion of the revision of SQA 119, this was identified as .

IFI 327/85-21-02, 328/85-21-02,

2. Corrective Actions Taken The SQN Standard Practice SQA 119 was revised with Revision 6, dated September 26, 1986, to provide the additional guidance.

Step 4.2.2.B.6.0 was added to give guidance in the evaluation for effects on equipment qualification. ECTG intervieumd SQN's Environmental Qualifications group supervisor, who stated that the revised SQA 119 provides adequate guidance.

3. Verification Methodology The SWEC concern identified for ECTG verification was stated as follows:

ECTG reviewed the Sequoyah Compliance Licensing files for internal and external correspondence related to this issue, the applicable procedure, a.ad the NRC status and tracking system for the issue. This review of the pertinent documentation, in addition to discussions with SQN's Environmental Qualification personnel, formed the basis for this verification activity.

4. V5rification Analysis The ECTG review of the pertinent documents indicate that the corrective actions outlined in II.B.2, above, have been completed and are adequate to resolve the concern and IFI. At the time of this verification, the NRC IFI remains open.

Corrective actions have been completed, and the issue is considered by SQN to be ready for NRC closure.

5. Completion Status Based on the verification activity above, ECTG considers the corrective action taken by SQN complete and the IFI ready for NRC closure. Following the NRC closure, the SWEC concern may be closed.
