In the latter case, the notice shall include the cause of noncompliance, av remedial actions taken, and the probability of meeting the neat scheduled requirements.

14. Recording of Results For each measurement or sample taken pursuant to the regatrements of this perett, the perutttee shall sacord the following information:
a. The enact place, date, and time of sampling;

( b. The dates the analyses were performed;

c. The persontslwho perforued the analyses;
d. The analytical techniques or methods used; and
e. The results of all required analyses.
15. Additional Monitoring by peretttee If the perutttee monitors ary pollutart at the location (s) designated herein more frequently than reqvf red by this permit, using approved analytical methods as spect(fed in Chapters M 214 and MR 219. the results of such monitoring shall be included in the Discharge lionitoring Report Form 3200-23, 3200-40, or av ether form as designated by tne Department. Such increased frequency shall aise be indicated.

18. Confidential Infomation Encept for data deterwined to be confidential under Section 147.0S(2)(c). Wisconsin Statutes, all sonitoring reports requitvd by this permit shall be available for pubitc f aspection at the headquarters of the Department of Natural Resevrtes. Knowingly making av false statement en av such report any result in the foposition of criminal penalties as provided for in Section 147.21. Wisconsin Statutes.
19. Transfer of control of Operation In the event of a transfer of control of operetton of the fact 18ty from editch the perettted discharges emanate to a person not named on page one (1) of this perett the permittee prior to such transfer shall nottfy his successor by letter that this permit is not transferable and that prior to ag discharge af ter such transfer, a new WPDES permit must be obtained. A copy of this letter shall be forwerded to the Department of Natural Resources. IFDES perett Section, tes 7921. Madison. Wisconsin $3707.

21. Ncncomp1tance Notifiestfon If. a. A description of the discharge and cause of noncompliance; and
b. An identification of the period of noncogliance, including esact dates and slees; or, if continuing. the anticipated time the noncompilance is espected to continue, and a description of the steps being taken to reduce, elleinste and prevent recurrence of the noncompilance.
22. Fact 11ttes Operation The permittee snell at eil times natntain in good working order and operate as efficiently as possible all treatment or control facilities or systems installed or used by the permittee to achieve compliance with tne terms and conditions of this perutt.

' 23. Bypassing Any diversion from or bypass of facilities necessary to estatain cogliance with the terms and conditions

' of this permit is prohibited, except (1) en inadvertent bypass lasting ne longer than one hour and l resulting from equipment desage or temporary power interruption, or (11) an unavoidable bypass necessary to

( prevent loss of itfe or severe property damage, or (118) a bypass of escessive store drainage or runoff whicn would damage any facilities necessary for compliance with the effluent limitations and prohibitions of this permit. In the event of occurrence of a bypass described by (11) er (till the permittee shall lunedf ately notify the Department District Office by telephone of such occurrence. In addition. the permittee shall notify the Department of Natural Resources. , , 24. Power Failures i / In order to maintain compliance with the effluent limitations and prohibitions of this perett fu the event

\ of the reduction. loss, or failure of one or more of the primary sources of power to the wastewater contr31 factittles. the permittee shall either:

a. provide an alternative power source sufficient to operate the wastewater treatment or control facilities if required by the Schedule of Cog 11ance contained in Part II. or,
b. If there is no such requirement, halt. reduce or otherwise control production and/or all discharges from the facility.
25. Definition Terms (1) For those terms used in Parts I and !! which arv defined in Section 147.015 Wisconsin Statutes, the meanings of that Section shall apply.

(11) As used in Dally Effluent Liettations. Part 11:

(1) 'Averige means the arittenetic mean of dally monitoring test results obtained during a calender month unless a different time period is specified for the given parameter; that is the total of such daily .tsults, in pounds per de, stiligrams per liter, or other appropriate unit divided by the neber of des for wntch results are totaled. (7) Discharge to the land disposal system shall be limited so that during surface spreading all of the sludge and any precipitation falling onte or flowing onto the disposal field shall not overflow the perimeter of the system.

(8) Vehicles used for transporting and spreading the sludge stall be designed to be kept tightly closed to prevent spillage or escape of odors while in transit or starage'.

(9) Sludge may not be land spread on land alth a slope greater than 125. During the period from December 15 through March 31 sludge may not be land spread on land with a slope greater than 65 unless the westos are Injected lessedletely Into the soll'.

fle) The vehicle used for land spreading the sludge shall be moving forward at all times when discharge is occurring unless the ' vehicle is equipped with a high pressure spray nozzle which is used to evenly distribute the wastes over the land. In addition the vehicle shall be equipped with a distributton system having the ability to spread the weste evenly over a width of 5 feet or greater.

(15) The permitten may develop'edditional lord spreading sites other then those Identified, provided they are approved in writing by this Department *, The Department may issue a vrltten notice to withdrew approval to use any site that is found to be enviroteientally unacceptable. This written approval or withdrawal shall be deemed as authority to use or reject the site until such flaae as the peruf t can be modified in ec.cordence with Section 147'.03, Wisconsin Statutes.
