On February 19, 1992, Wisconsin Electric notified the Department that the daily maximum total residual chlorine (TRC) effluent limitation was exceeded at Point teach Nuclear Plant. The TRC concentration in the Unit 2 cooling water discharge (outfall 002) >

Was 0.73 mg/l on February 14, 1992.

Cause of Incident i The incident was cause? by a lack of decnlorination dilution  :

water flow which in necessary to carry the dechlorination chemical (sodium bisulfite) to the condenser water box outlets prior to final discharge. The dechlorination water flow was not present because an isolation valve was inadvertently left shut following maintenance work on the dechlorination system. .

I Corrective Action Taken During chlorination of the. Unit 2 condenser, the dechlorination ,

dilution water problem was diagnosed and remedied. After the dilution water was restored,-a condenser cooling water discharge sample from outfall 002 was analyzed and found to have a concentration-of TRC less than 0.01 mg/1. The sample result verified that the dochlorination system was operating properly.

  1. Action Taken to Prevent Recurrence of Incident A formal change is being made to Point Beach Nuclear Plant's Chemistry Analytical Methods and Procedures (" CAMP 009"). A note in the procedure vill direct the system operator to review a checklist of valves for the chlorination and dathlorination system to ensure all valves are returned to proper setting following synnem maintenance.
