WPDES PERMIT WI-0000957-4 P_QIliT. BEAC11 NUCLEAR PLANT On March 6, 1992,_ Wisconsin Electric notified the Department that the daily-maximum total residual chlorine (TRC) effluent limitation was exceeded at the Point Beach Nuclear Plant (PBNP).

The;TRC' concentration in the Unit 1-cooling water discharge (outfall 001) was 0.-71 mg/1-on March 4, 1992.
Cause of Incidqtt The limitation was caused by a malfunction of the chlorination
. pump stroke adjustment control which allowed the rate of sodium hypochlorite addition to increase in) the maximua pumping rate.
The amount of dechlorination chemical-was not sufficient to.
reduce the TRC below the permit limit of 0.2 mg/1.
Corrective Action Taken to Prevent Recurrence of Incident A maintenance work' request-was initiated to repair the pump stroke adjustment control.
In the interim, the stroke _ adjustment control: has been tightly locked to _ hecp it from changing during pump operation.
Also, a formal change is being made to PBNP's Chemical Analytical Methods.and1 Procedures-(" CAMP 009") to ensure ithat the.systen. operator verifies:that the adjustment-control is
-locked.
The system has operated within permit limits since the l
-March 4,_1992 TRC limitation exceedance.
