The enclosed attachments provide the information requested in your April 17, 1986 Supplemental Safety Evaluation (SSE) on the Detailed Control Room Design Review (NUREG-0737, Item I.D.1) for the Trojan Nuclear Plant.

Attachment A provides responses to the general issues raised in the SSE.

Attachment B provides revised pages for Amendment 3 of Topical Report PGE-1041, "DCRDR Summary Report", Volume 2.

I PGE RESPONSE TO DETAILED CONTROL DESIGN y

REVIEW SUPPLEMENTARY SAFETY EVALUATION Gdalitications and Structure of the DCaaR Team: 1 Human factors specialists continue to be involved in activities related to the DCRDR. PGE initiated a continuing services contract with General Physics Corporation (GPC) so that human factors expertise would be avail-able upon request.

Human factors specialists were not involved in the comparison of tasks and information and control requirements between the Revision 1 and Revision O Emergency Response Guidelines. A member of the DCRDR team performed these reviews and the verification of the controls and displays required to satisfy their requirements. Based upon his experience in the DCRDR as well as Trojan's validation and verification of the plant-specific Emergency Operating Procedures, assistance from a human factors specialist was not needed.

Human factors specialists have been involved and will continue to be involved in the annunciator system upgrade effort.

The selection of design improvements was a process in which human factors specialists were always involved. Conclusions reached during the meter study were discussed with GPC's specialist. For those instances in which we are unable to modify the control room as originally indicated at the completion of the DCRDR, GPC's specialist is consulted to discuss alter-native resolutions.

The verification of design improvements has had some -involvement by the

GPC specialist. In particular, last November while onsite for the SPDS human factors evaluation, a GPC specialist reviewed the surface enhance-ments made to the control boards as well as specific design changes installed.

Finally, PGE has issued a Human Factors Design Criteria and Review Program document. That document was critiqued several times by GPC's specialist for its human factors content.

Control Room Survey:

The objective of the annunciator study was to determine whether condi-tions warranted system replacement or selective modifications to resolve HEDs. The completion of the study resulted in selection of the replace-ment alternative. That decision was made in December 1985 and subse-quently the design group initiated the work required to develop the 1 preliminary and detailed engineering plans. These tasks include alarm l

Trojan Nuclear Plant Mr. S. A. Varga Docket 50-344 Attachment A License NPF-1 June 24, 1986 Page 2 of 6 reduction and prioritization and improvement in interpretability and arrangement as well as in-depth review of the system installation requirements.

Human factors principles have been part of the annunciator system upgrade design criteria from the beginning of the review. The intent is to re-solve, as much as possible, all HEDs in accordance with accepted human factors guidelines provided either by NRC or industry standards. GPC has been tasked to review the annunciator system upgrade study and provide a proposal to perform a review of the new annunciator system design.

While PGE cannot state at this time exactly how each individual HED will be corrected by the annunciator system replacement, we can generally pre-dict that the effects on HED resolutions will be as shown in Table 1.

Prior to approval of the final design, mockups of the annunciator panels will be constructed so that arrangement and interpretability resolutions can be approved by human factors engineering and management personnel.

Selection of Design Improvements:

Responses to comments in the SAIC Technical Evaluation Report (TER) on specific HEDs are included as revision to the individual HED forms, PGE-1041, Volume 2. As a group they make up Attachment B to this letter.

Numerous comments (HEDs 298, 299, 301-305, 307-309, and 311) concerning the P-250 computer design and operating characteristics were received.

Responses'to each are given on the applicable HED forms. In general, the P-250 provides display, alarming, data logging and some engineering cal-culations. There are no process control functions associated with the computer and it is not required for Plant operation. Since the P-250 is not safety-related nor the primary indicator of Plant status, we do not foresee any adverse impact upon Plant operations by the use of the P-250.

The SAIC TER Summary and Conclusions states that "Through the revisions in Amendment 2 to the description of the " ongoing human factors mainte-nance plan" and in HED descriptions and resolutions, PCE has indicated that it is no longer including the remote shutdown panels in the DCRDR". The removal of the remote shutdown panel from the DCRDR was described in Amendment 1 submitted on April 17, 1985. Amendment 1 was subsequently referenced in the original DCRDR Safety Evaluation of July 24, 1985.

HED Forms 470, 471 and 472 are indicated not to have been found in Amendment 2 received by SAIC. PGE verified that in-house data sets made in the same production run as those sent to the NRC did include these J

- l forms. Therefore, we believe they were lost in transmittal between the NRC and its subcontractor. In any event, they are included in this sub-mittal.

As stated in the Summary Report, PGE's rationale for scheduling modifica-tions not only took into account the HED category, but also complexity of the solution, time for preliminary and detailed design work, procurement of material, and installation. In general, modifications not only re-solve DCRDR issues, but also affect other issues such as environmental qualification, fire protection, etc. Resource loading requires phased implementation of planned changes. While the company has attempted to be as responsive as possible, not all changes could be made in the first one or two outages after the study.

Please note that several HEDs with implementation dates of concern to the staff have been completed ahead of schedule. The original dates, se-lected in 1984, were best estimates, but where feasible, acceleration of the work has occurred. This is the case for HEDs 1, 55, 57, 115, 120, and 131 from Appendix D of the TER. The majority of other HEDs listed in that appendix are part of the annunciator system upgrade effort which is addressed above. .

Except for the annunciator system replacement, verification will occur i after, rather than before installation. Thus PGE recognizes that the verification process must continae beyond implementation to assure our-selves that resolution has been achieved and no new HEDs created. >

Section 6.0 of PGE's Human Factors Design Criteria and Review Program

document provides the methods utilized to accomplish this
surveys of j features, task analysis, verification of suitability and availability and J

validation of performance capability. One or more of the methods may be required for a given change.

i We would prefer not to leave the DCRDR open until such verfications are

, complete. Such items could be referred to the NRC regional office to verify completion as part of their routine plant inspections. Our desire is to conclude the DCRDR phase of our human factors effort at the earliest possible date.

Coordination of the DCRDR With Other Improvement Programs:

Specific response to the concerns on HED 403 is provided on the HED form. Operator training is clear that plant safety is the operator's primary responsibility and production-related activities are secondary.

Hence, it is understood that administration of tag-outs, when a clearance operator is not-stationed, is a secondary duty.

Prior to return to power, all operating crews and shift technical advisors attend " Ready for Startup" training. Lectures are given on all design changes, including the control room. Important design changes installed during the operating cycle may be addressed during periodic retraining sessions when necessary.

RATHER, THE OVERSPEED TRIP TEST BUTTON IN THE CONTROL ROOH IS USED TC TEST THE OVERSPEED DEVICE WITHOUT ACTUALLY CAUSING A HFP TRIP. FOR THAT BUTTON TO HAVE ANY EFFECT, THE "0VERSPEED BLOCK" PUSHBUTTON IMMEDIATELY TO THE LEFT OF THE "0VERSPEED TRIP" PUSHSUTTON HUST FIRST BE DEPRESSED. THEN THE (3)

OVERSPEED TRIP TEST CONTINUES BY DEPRESSING THE OVERSPEED TRIP PUSHBUTTON.

ACTUAL CONTROL VALVE CLOSURE IS PREVENTED. THEREFORE, INSTALLATION OF A RED COLLAR DEVICE IS NOT HARRANTED.
