I 2.2 DEFINITIONS l

2 of 4 3.0 PROCEDURE 3.1 INSPECTION FREQUENCY AND SCOPE Frequencies of inspections shall be determined jointly by the Quality. Control Supervisor and the Quality Engineering Group based on field installation activities.
These inspections shall be accanplised by assigned Inspectors in accordance with the established frequency.
Inspection frequencies for va ri ous elect rical,
instrument and mechanical applications va ry; the f ollowing a re the mqui red minimum frequencies for the various activities:
a 3.1.1 Installation Surveillance of installation activities shall be a minimum 1
of ten (10) bolts per discipline per shift, or at the frequency di rected by the Quality Control Supervisor to verify a high confidence level.
3.1.2 Abandoned Bolt Holes Abandoned bol t hole patching will be verified by the QC Inspector on a frequency conmensurate with constnJcti on activities as detennined by the Quality Control Supervisor /

Torquing requi rements for the va'rious discipline applica-tions shall be as follows:
A.
For all supports (mechanical) and cable tray supports (pipe electrical) at least one (1) concrete anchor bolt per base plate shall be witnessed.
B.
Whe re electrical boxes (this includes fixture boxes) mount directly to building concrete, or where conduit or tubing is mounted to the building strJCture with one (1) hole straps, the inspection / witness attribute shall be accanplished on one (1) bolt for each fixture per unit, section or assembly.
C.
Whe re concrete anchors are used to mojnt electrical i
equipment
- panels, junction boxe,
ins trument and condui t supports, one (1) anchor bolt per unit or assembly shall be witnessed.

Lighting fixtu res requi ring seismic restraints shall have at least one (1) bolt on each seisraic bracket wi tnessed.
Other itens not identified above that require inspection of concrete anchor bolts will be evaluated and a frequency conmensurate with the application will be assigned as per this section.
3.2 INSPECTION PLANNING Quality Engineering working jointly with assigned Quality Control (QC) personnel shall plan inspections to result ir maximum utiliza tion of qualified
'manpowe r wi thout conpronising the. quality objectives as stated in Section 3.1 ab ove. Those planning efforts shall result in an Inspection Report (IR) per the provisions of Reference 1-A detailing the itens to be inspected.
3.3 INSPECTION TECHNIQUES Quality Cont rol (QC) pe rs onnel shall acconplish the inspection efforts as directed by the Quality Control Supervisor or his designee using surveillance witnessing and/or monitoring techniques.
These techniques will consist of, but are not necessarily limited t o, the use of precision measuring devices to physically measure the bolt or hole, witnessing of drilling and installation activities and post installation measu ring activities to ve ri fy ins tallation pa ramete rs.
Results shall be documented on the IR in sufficient detail to pe nnit evalua tion of the quality of the ins tallati on efforts without recou rsa to the Inspector.
The Inspector shall canplete and attach supplementary sheets to the IR if necessa ry to achieve the objective.
The Inspection Attributes shown on the IR are the minimum inspections to be accanplished and may be expanded if installation conditions warrant with the approval of the Quality Control Supervisor or his designee.
NOTE:
In the past, sone inspections have been documented on the Operation Traveler.

AUG 11 681 4 of 4 3.4 NONCONFORMANCES AND DEFICIENCIES Installation efforts dete nnined to be inconsistent with established ins tallation requi rements and acceptance criteria shall be documented on the IR.
If the discrepancy can be cormcted in a reasonable pe ri od of time, as dete nnined by the QC Inspector, then work may continue after corrections art made, provided the work conforms to the installation require:nents and is witnessed by the QC Inspector.
If the noted discrepancies car..not be made to confonn t5 installation ' requirements and witnessed by the Inspector, the provisions of Reference 1-8 and 1-C should be.
implemented as necessa ry to control further proces sing.
The IR shall then be elosed and processed as stated in Refennce 1-A.
3.5 DOCUMENTATION t
Inspection Reports and/or Quality Ins tructions shall be prepared and issued delineating the methaf(s) of inspection and applicable criteria to be used in perfonning that inspection.
Quality Instructions detail the documentation requirtd to be canpleted to provide abjective evidence of compliance with specified Engineering / Construction criteria.
These reconis shall be submitted to the Quality Control Supe rvisor or his designee upon conpletion for review, processing and filing in acconfance with CPSES requirements for QA Reconfs.
