513.2 GENERAL In addition to the procedures and standards outlined herein, all procedures of Robert W. Hunt Company Nuclear Quality Assurance Manual (NQA) as. approved by the client for the project are applicable.
513.3 REVISIONS Except for.the procedures contained in Appendix C, revisions to this manual shall be made as outlined in NQA 3.3 except that individual pages of this manual may be revised without revising he entire manual.

513.9 CONCRETE TESTING AND CONTROL 513.9.1 MIX PROPORTIONS Concrete mix proportions shall be determined by a Hunt Level III Concrete Examiner.

A Comanche Peak Mix Number beginning with the prefix HCP and ending with a number.
The numbers shall be assigned consecutively.

HCP 2 (The second Comanche Peak Mix by Hunt)
HCP 36 (The 36th Comanche Peak Mix by Hunt)

- 513.9.2 CONCRETE IN PROCESS TESTING a)
Acceptance concrete temperature measurements (Hunt Procedure C1012) shall be made as follows:

All acceptance concrete temperature measure-ments shall be made at the location of concrete discharge into the forms.
2)
One acceptance temperature test shall be made for each set of strength tests from the same batch for strength tests.

Communication between the test lab and batch plant will be maintained to report results of slump,' air content, and temperature tests.
Water added after it is in the mix truck is done in the presence of a R.W. Hunt test lab representative.
Water added can't exceed the H O cement ratio and water meters 2
must be calibrated.
The test lab representative will verify this and sign or initial the delivery ticket.
After water addition, an additional 30 revolutions will be turned before resampling or placing.
e)
Whenever tests are made, the results shall be compared with the mix requirements shown on the Concrete Design Mix Number List and in Table 5, and the concrete represented by the tests shall be accepted or rejected as follows:
1)
Concrete shall be accepted if all fresh concrete tests, slump, air, unit weight, temperature are within the limits shown on the Concrete Number Mix List.-

All standard cured strength specimens shall be tested in accordance with Hunt Procedures E1001.

513.9.3 TESTIflG CONCRETE RElilFORCIllG BARS (Test only. performed when specifically requested by Brown & Root)
Concrete Reinforcing Bars shall be tested in accordance with Hunt Procedures U1069 and reported on Form HCP-U1069 by an inspector with certification of Level I or higher.

513.9.4 TESTIllG MECHAtlICAL REINFORCING BAR SPLICES Mechanical reinforcing splices shall be tested and reported in accordance with Hunt Procedure U1091, by an inspector with certification of Level I or higher.

-CALIBRATION a) Test equipment shall be calibrated using methods and at frequencies listed in Table 7.

b) Calibration reco.rds shall be in accordance with NQA 7.1, 7.2.
c) Calibration of tools and equipment can be performed by the level of inspector who performed the tests that the-equipment is used for unless a higher level of certification of exper-ience is delineated elsewhere in our program.
513.11 STORAGE & HANDLING OF DOCUMENTS PRIOR'TO SUBMITTAL OR NOT FOR SUBMITTAL TO BROWN & ROOT a) All site documents for which there are not copies filed in a separate remote location, must be stored-in a fire resistant catinet. The cabinet must have a one hour minimum rating.
b) Specific items requiring fire resistance storage, and the specific items requiring fire resistance storage and the ecific method for handling these items are described below:

Cylinder Data Cards for concrete cylinders in storage.
While cylinders are being cured, their corre-sponding Cylinder Data Cards shall be filed in a plain metal storage. box and they shall be filed according to their date of test.
The' metal box containing the cards shall be stored in the fire resistant cabinet, except when it must be used for filing, etc.
When test cylinders are to be tested, their corresponding data cards shall be transferred from the main file box to a temporary metal container.
The main file box shall be returned to the fire resistant cabinet.
The temporary box with the necessary data cards are for use in recording casults during testing.
After cylinder test results for a particular data card have been reported, the data card is not required to be filed in the fire resistant cabinet.
2)
Worksheets for materials tests, etc.
Worksheets for tests being performed (not blank worksheets)- shall be stored in the fire resistant cabinet except for the actual times that it.is necessary to have the sheets in the lab for data recording.
While the sheets are in the lab, they shall be kept in folders or binders.
No loose papers are allowed in the laboratory.
After the test results for a worksheet have been transmitted to Brown & Root, the Hunt copies may be stored in regular (not 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br /> fire rated) cabinets.

Calibration record, cards shall be stored in the fire resistant cabinet.

Record retention times (in accordance with ANSI N45. 3. 9 Draft 11 Rev. 0) shall be stated on Hunt transmittal forms to assist Brown & Root, and Texas Utilities Service maintain their Quality Assurance Records.
Required record' retention times are listed in Table 8.
g) Hunt is not required to retain its record copies (after submittal of reports, etc. to Brown & Root) in accordance with the retention time requirements of ANSI N45. 2. 9 Draft 11 Rev. O h) Hunt will retain its project files until the time of Hunt's project demobilization.
513.13 DISCOVERED NON-CONFORMITIES Whenever, during testing or inspection, Hunt discovers a non-compliance with project requirements, the non-compliance shall immediately be reported first verbally and immediately thereafter in report form directly to the Brown & Root Project Q. A. Manager or his designated representative.

.G&H SPEC..SS-ll INTERPRETATION Per request of B&R QA/C a review of paragraphs 4.2 and 6.3 was per-formed to determine the acceptance and rejection criteria for location of c::dweld sleeves.
Through consultation with the on-site G&H civil engineers and my own interpretation, the following criteria was arrived at and same was relayed to B&R QA/C (Geo. Fisher).
Paragraph 4.2 provides a 12"i 1/4" dinension for marking a reference m2rk from the end of each bar to be spliced.
The referenced Erico Products ')
Catalog RB 20M-173 allows a maximum gap between bars of 3/8".
Adding all m ximum allowed dimensions together allows a 24 7/8" maximum dimension be tween reference marks.
Taking the minimum allowed dimensions (11 3/4" from.
bar ends to reference mark and 3/16". minimum gap between bars),.the minimum..i.'
dimension between bars is 23 11/16".
Paragraph 6.3 requires the spliced sleeve to be centered within 1/2"
