Dear Sirs:

We would like to report an incidence of non-compliance with the Technical Specifications for the Dow TRIGA reactor which occurred on January 6, 1976 at our facility:

After completing the daily startup check which requires the testing of the scram circuits and control rod interlocks, etc., without taking the reactor to critical, the operator activated the manual scram circuit to secure the reactor.

He verified the control-rod drive motors had moved to their "down" position.

He then proceeded to calibrate the recorder display at the console and left momentarily to the adjoining laboratory to prepare for the planned sample activation.

Through an oversight he left the console key in the switch, so that the reactor was not secured in terms of the Technical Specifications.

After an elapsed time of approximately five minutes, the fact that the key was still in the console was recognized by a cc-worker, who notified the operator, who thereupon with-drew the key from the console and secured it in the proper way.

The incident involved no damage to property nor any radia-tion exposure to personnel or members of the public.

It involved no release of radioactive material or any mechanical or electric mal-functioning of the facility.

As remedial action, the incident was discussed with the operator and brought to the attention of other operators in the facility.
