f LOSS OF RHR SEQUENCE OF EVENTS Saturday night, October 26, 1991.

Reactor cavity draining in progress per DOP 12000-C and SOP 13033-1.

RHR

'B' aligned for coolant circulation & cooling and providing letdown to VCT.

RHR

'A' aligned for draining cavity to RWST.

At approximately 197' elevation as indi-cated on 1-LG-10401, cavity draining rate was decreased from approximately 2500 GPM to about 800 GPM.

At approx. 2200 hrs EDT received annunciator ALB 06 D03 "ACCUM i4 HI/LO LEVEL".

I observed lLI-957 indicating at top of scale (100%).

I then tapped on lLI-957 and it's indi-cation decreased to 60%.

Mike Henry then closed 1-HV-8716A and opened 1HV-8809A.

Site glass indication was reported as approx. 194', tygon hose indicated 194' and visual check was reported as level at Rx vessel flange.

C. Ladd, M. Henry and myself, after consulting I&C, discussed the validity of ILI-957 indication.

We then decided to reinitiate flow and continue draining to 192' elev.

M. Henry reopened lHV-8716A, closed g

At approx. 2235 hrs EDT I observed RHR pump

'B' flow oscillation and pressure de-

As flow & pressure decreased to zero GPM/

PSIG, I closed lHV-607 and 1FV-619 resulting in RHR

'B' running o& mini-flow.

M. Henry then closed lHV-8716A and we realigned RHR

'A' to refill the cavity from the RWST.

At this time lLI-957 was indicating approx. 32%.

Vessel was reported as approx. l' below the flange.

Site glass indi-j {,

6 cation reported as 193'.

As vessel level increased RHR

'B' pressure began to increase.

At approximately g

193'4" site glass indication, I restored RHR

'B' flow to approx. 1000'UPM.

reported no indications of cavitation at RHR l

pump 'B'._

we stopped filling cavity from RWST via RHR

'A' at. approx. 194' per site glass.

At approx. 2310 hrs cavity level reported as stable at 194'5".

At 2316 hrs RHR

'B' flow was restored to 3000 GPM.
