APPENDIX A NOTICE Of VIOLATION TU Electric Docket '40. 50-445/90-47 Comanche Peak Steam Electric Station. Unit 1 License No. hPF-87 During an NRC review completed on February 21, 1991, a violation or NRC requirements was identified.
In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1990),theviolationislistedbelow:
Criterion XV.; of 10 CFR Part 50, Appendix 8. " Corrective Action," statet, in part, that " measures shall be established to assure that conditions adverse to quality,-such as failures, malfunctions, deficiencies, deviations, defective material and equipment, and nonconformances, are promptly identified ano corrected."
Contrary to the above, established measures did not assure that deficiencies with safety-rel?ted instrumentation loop scaling docurrentation were promptly identified and corrected. Significant involvement from a former employee over an extended period was required before deficiencies were identified and ultimately corrected. The fonner employee initially identified design-related scaling concerns in the 1986/1987 timeframe.
These concerns fonned the basis for the documented deficiencies identifica in a letter (NE-19097) from TU Electric, Engineering, to Stone and Webster Engineering Corporation, dated May 10, 1988,
Subject:
Comanche Peak Steam Electric Station Resolution of Technical Concerns on Westinghouse 7300 Systems. As of August 1989, the scaling documentation for CPSES, Unit 1, contained hundreds of errors, omissions, discrepancies, and deficiencies.
Actions to ioentify and correct deficiencies with scaling documentation were completed in 1990.
This is a Severity Level IV violation.
