Comanche Peak Steam Electric Station Permits: CPPR-126 Unit 1, Glen Rose, Texas During an NRC inspection conducted on August 7-11, 1989, a violation of NRC requirements was identified.

in accordance with the " General Statement of-Policy and-Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1988),-the violation is listed below:

Criterion V of Appendix 0 to 10 CFR Part 50, as implenented by Section 5.0, Revision 3, of the TV Electric Quality Assurance Plan (QAP) states, in part, that activities affecting quality shall be prescribed by documented instructions,

' procedures, or drawings of a type appropriate to the circumstances, and shall

.be accomplished in accordance with these instructions, procedures, or drawings.

Electrical Erection Specification 2323-ES-100, Rev'ision 6 requires that bolted electric "V" splices made with Okonite tape should be constructed such that the

.T-95 insulating tape and T-35 jacketing tape are applied continuously from one g

cable, over the bolted connection and continuing onto the second conductor i-

-following the "V" configuration.

Contrary to the above, the tape splices in Limitorque motor valve operator

_ Number 1-8808C were not installed in accordance with the above requirements.

In these splices, the T-35 jacketing tape was applied by wrapping the tape L.

around both conductors at the same time,'therefore not providing a smooth surface on one-conductor, over the bolted connection, and then along the second conductor.

L This is a Severity Level IV violation (Supplement I) (50-445/8660-V-02).

Pursuant to the provisions of 10 CFR 2.201, TU Electric is hereby required to submit a written statement or explanation to the U. S. Nuclear Regulatory Document Control Desk, Washington, DC, 20555, with a copy Commission, ATTH:

Division, Office of Nuclear Reactor Regulation, within 30 days of the date of the letter transmitting this Notice. This reply should be clearly marked as a " Reply to a Notice of Violation" and should include for each violation: (1) the reason for the violation if admitted, (2) the corrective steps taken and the _results achieved, (3) the corrective steps that will be taken to avoid l

further violations, and (4) the date when full compliance will be achieved.

If an adequate reply is not received within the time specified in this Notice, 1
