The investigation was initiated by CO I on February 23, 1959, when Norman C. Moseley, Senior Reactor Inspector, CO !,
telephoned Calla at his of fice to chtain specific details of the allegttien in Kandle's letter to CO:I.
Cella, who declined to be specific, made the following statements concerning CC-1 and other nuclear plants:
l.
Material war supplied, w__a the knowledge of subecntractors and fabricators, by firms who ignored purchase specifications and ASMI Codes.
2.
M..terial was supplied by firms who where nct qualified to manufacture it and were not distributors of any of the incwn qualified manufacturers.
3.
In so:;- cases, material certificates which were provided were falsii ed or improperly verified.
4.
Prior to placing orders with fabricators, inspections were nc:
made of the fabricators' shops or quality contro; programs.
In addition, Cella stated that a: CC '- 1, some pipe which was specified to be ASTM-A-312 was actually ASTX-A-310FX, which, according to Cella, was thin-walled and because of the uso of fillcr metal was e
unsuitable for nuclear applicaticns.

m In an interview with Cella on March 11, 1969, at his office in Dover, were informed cf the following additicnal New Jersey, Tillou and Rya: 4 allegationsa 1.
Burns & Roe (B&R) bought used valves for use at CC-1, had them t
rebuilt by a Clifton, New Jersey firm, a.d then requestad Cella, who owns Universal Testing Lab., Inc., at Cad : Greve, New Jersey, to test and certify the valves to specifica:icns.
Cella refused and the tests were done by an unidentified New York City testing organisation.
2.
General Electric Company (GE) tought piping material at a 40%
saving in cost in Japan wath no reliable material certifica:icas,
B&R bought piping material frcm Sandvick ?ipe and Tube Corpcration (Sandvick), Suoden, with no reliable material certifica:icns.
3.
Ccnsolifated Edison had rejec:ad piping materini received fr:r Dravo Corporation for ea;ng thin-wa' led anc andcrv.ci; '.;, ana i

In the second case, Swepco Tube Corporation (Swepco),.Tillou exanined 14 radiegraphs (total) relating to three fittings, a shart ell, a long ell and a reducer. These radiographs we :e of excellent quality; they revealtd no defects. Also, t :e qualification record of the lone welder used for the work was in order. c, It is to be noted that if the requirements for final inspection of a fitting are those given in A-403, only 100% weld radiography, plus a limit on weld reinforcement, need be met on the finished product. How ever, since B&R specified seamless j fittings and many eventually were supplied as welded fittings, l there is a question as to which ASTM specification ' mast be satisfied. If the supplier reeotts to A-358 (welded pipe with filler metal) as his starting material, which is permitted as an alternate to A-312 (seamless pipe), then all testing requirements imposed by A-358, including 75% yield strength hydrostatic, face and root bend, andanalysis of weld metal must be conducted. These tests would be performed on the starting materitl, not the formed fittin3 4. The firms that supplied the pipe material in question (presumably A-312FM) were neither qualified to manufacture it nor distributors of any known qualified manufacturers, a. Of the few systems chosen for analysis (see 1.a. above), CO has found, deductively, that only one firm. Alloy, could be the subject of the aAlegation. The other firms involved, Albert and Tubeco, are either a recognized supplier (Albert) er pipe fabricators (Tubeco). b. during a visit to Alloy on April 8, 1969, was told by Sta .,g, former General Manager for Alloy, that the firm is today, as it was two years ago, incapable of manu f acturing welded pipe with wall thickness greater than 0.375" without the addition of filler metal. All 6", 8" and 10" pipe in question has wall thicknesses of 0.378" or greater. Therefero, it can be concluded that Alloy was not a qualified manutacturer of nonfiller metal pipe of the sizes ordered by Albert. Note: Tillou has determined that with special equipment, A-312 pipe can be manufactured today without the use of filler metal to thickness not exceeding 0.500". c. A related allegation was that " material was supplied by firms who ignored purchase specifications and ASME codes. This was done with the knowledge of subcontractors and fabricators." (Alleged by cella to Moseley on February 28, 1969.) This complaint could relate directly to the fact that Tubeco's orcer demanded manufacture of pipe that was impossible for Alloy to fulfill. Addition of the requirement for 100% radiography of the longitudinal weld seams invites the conclusion that there was a reluctance on Tubeco's part to order the pipe to the A-358 Code, which recuires additional tests (weld metal analysis, face and root head tests, 75% yinld s tre ngth hydros tat ic, etc.) u wwaa;n-qq*1:p '.
