- VTVYS1 05000271 LER 78-31/3L EVENT DESCRIPTION AND PROBABLE CONSEQUENCES During the weekly review of completed surveillance tests, following the plant startup of October 13, 1978, it was discovered that the monthly 4 hydrogen detector functional test as required by Technical Specification 3.9.2.b was not performed. The test cannot be accomplished without pro-cess steam and should have been completed upon returning to power opera-tions.
Since the functional test was performed immediately following dis-covery and completed satisfactorily, it is concluded that there were no
. consequences or potential consequences to the health and safety of the public. There have been no previous reportable occurrences of this type.
CAUSE DESCRIPTION AND CORRECTIVE ACTIONS c The test was overlooked because of the activities associated with the plant startup and return to power. The functional test was success- -
fully completed on October 16, 1978, and will be conducted in accordance with the surveillance schedule in the future.
