{{#Wiki_filter:- - t + + i I APPENDIX' A l NOTICE OF VIOLATION I Philadelphia Electric Company Docket No. 50-278 -I Peach Bottom Atomic Power Station Unit 3 License No. DPR-56 Delta, PA j During an NRC inspection conducted on September 14, through October 30,1993, a violation l of NRC requirements was identified. In accordance with the " General Statement of Policy and l Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1992), the violation is listed below: i Technical Specif. cation 6.8.1 requires that written procedures and policies be established and implemented that meet the requirements of Sections 5.1 and 5.3 of ANSI N18.7-1972. ANSI 18.7-1972, Section 5.1.2 requires that procedures be followed, the requirements for use be l i prescribed in writing, and that rules be established that provide methods by which temporary changes to approved procedures can be made. The following example of failure to adhere to { these requirements was identified. i Administrative Procedure A-4.2, " Station Quali5ed Reviewer Program," Revision 1, l Section 7.1.4, requires that the Station Quali6ed Reviewer (SQR) shall review the j package for technical accuracy and adherence to quality program requirements. The-l Nuclear Quality Assurance Plan, Surveillance Testing Section 11.4 requires that shift operations personnel shall have overall control of the tests affecting plant operations, to 3-assure that testing systems do not adversely affect the safe operation of the plant. j Contrary to the above, on October 19, 1993, a Station Quali6ed Reviewer did not perform an adequate technical review of a temporary change to routine test procedure, i RT X013 210.3, " Reactor Core Isolation Cooling (RCIC) Overspeed Trip Test." Further, Operation's Personnel did not exercise control of the test by running the RCIC system with steam while a piping breach existed. As a result, an inadvertent release of j radioactive contamination into the Unit 3 reactor building occurred. i This is a Severity Level IV Violation (Supplement I). Pursuant to the provisions of 10 CFR 2.201, Philadelphia Electric Company is hereby required j to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, A'ITN: Document Control Desk, Washington, D.C. 20555 with a copy to the Regional -{ Administrator, Region I, and a copy to the NRC Resident Inspector at the facility that is the _ subject of this Notice, within 30 days of the date of receipt of the letter transmitting this Notice l of Violation (Notice). This reply should be clearly marked as a " Reply to a Notice of Viola- }' tion" and should include for each violation: (1) the reason for the violation, or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date l when full compliance will be achieved.
