ENCLOSURE 1 NOTICE OF VIOLATION PECO Energy Company Docket Nos. 50-277,50-278 Peach Bottom Atomic Power Station Units 2 & 3 License No.a. DPR-44, DPR-56 Delta, PA During an NRC inspection conducted from September 22,1998, through November 9, 1998, one violation of NRC requirements was identified. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," NUREG-1600, this violation is listed below.
Technical Specification 5.4.1 requires, in part, that written procedures be established, l
implemented, and maintained covering the applicable procedures recommended in Regulatory Guide 1.33, Appendix A, November 1972. The procedures listed in Regulatory Guide 1.33, Appendix A, include surveillance tests for process radiation monitoring calibrations.
Contrary to the above, on August 10,1998, Peach Bottom Chemistry technicians failed to l
properly implement Step 4.4.3 of ST-C-095-878-2," Refuel Floor Vent Exhaust Rad Monitor Calibration and Functional Test for RIS-2-17-458A and C." Specifically, Step 4.4.3 required the technicians to stop the test and notify the reactor operator or Shift Menager and Chemistry Supervision and request permission to continue to the next applicab!e step if an unexpected response was produced during the test. The technicians failed to perform the required actions of Step 4.4.3 when the 'D' detector, RIS-2-17-458D went downscale during the test. The downscale reading after the 'D' detector was inadvertently removed and dropped, damaging the detector.
This is a Severity Level IV violation (Supplement 1).
The NRC has concluded that the information regarding the reason for the violation, the actions taken and planned to correct the violation and prevent recurrence, and the date when full compliance was achieved is already adequately addressed in this inspection report. However, you are required to submit a written statement or explanation pursuant to 10 CFR 2.201 if the description therein does not accurately reflect your corrective actions or your position. In that case, or if you chose to respond, clearly mark your response as a " Reply to a Notice of Violation," and send it to the U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Region I, and a copy to the NRC Resident inspector at the facility that is subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice).
