COMBINED INSPECTION 50-277/93-24 AND 50-278/93-24
This letter transmits the findings of the resident safety inspection conducted by Messrs. W. L.
Schmidt, F. P. Bonnett, R. K. Lorson, and others from September 14 through October 30,
1993, at the Peach Bottom Atomic Power Station, Delta, Pennsylvania. The inspectors based
these findings on observation of activities, interviews, and document reviews, and discussed
them with Mr. G. Edwards of your staff.
Overall, your staff conducted normal operations and shutdown activities at both units safely.
Management demonstrated conservative attitudes toward safety when Unit 2 was shutdown
following your identification of cracking on safety related motor operated valve yokes at Unit
3 and following the identification and evaluation of cracks in the Unit 3 core shroud.
The operations staff performed well during routine evolutions; still, there were several instances
where individual poor performance caused some difficulties. Operators did not return the
control room ventilation system to a fully operable condition following a planned activity.
Refueling operators failed to evaluate a condition that led to a fuel bundle being stuck partially
,
inserted in a spent fuel rack. An operator caused the non-safety related reactor water cleanup
system pre-coat tank to overflow causing radioactive contamination of sections of the Unit 3
reactor building. The above instances were of minor safety significance, but showed the
necessity for more individual care and management supervision while conducting activities.
One event was of greater concem, a poor safety review of a temporary change to a reactor core
isolation cooling test procedure led to the inadvertent release of radioactive contamination within
the Unit 3 reactor building. While this resulted in a minor clothing contamination, our review
,
indicated poor management review and control of activities related to the specific testing. This
is a violation of NRC requirements, as specified in the enclosed Notice of Violation (Notice).
In your response, you should document the specific actions taken and any additional actions you
plan to prevent recurrence. In addition, in one instance, we have exercised enforcement discre-
tion because of the low safety significance of the event and your corrective actions met the
