APPENDIX A' NOTICE OF VIOLATION Philadelphia Electric Company Docket Nos. 50 -277/50-278 Peach Bottom Atomic Power Station, Units 2 &3 License Nos. OPR-44/DPR-56 As a result of the inspection conducted on September 26-30, 1988, and in accordance with the "General Statement of-Policy and Procedure for NRC Enforcement Actions," 10. CFR Part 2, Appendix C (Enforcement Policy) (1986), i the following violation was identified:

A. 10 CFR 50.49 paragraphs (f) and (g) require that each item of electrical I equipment important to safety be qualified and that qualification must be completed at a time no later than November 30, 1985. Contrary to the above;

1. On June 19, 1987, the inspector identified 6 wire crimps inside 2 Limitorque Valve Actuator housings (3 in M0-2-14-0050, 3 in  :

M0-2-14-0078), for which qualification was not demonstrated in that l the test report did not provide evidence that the wire crimp surface  :'

was in contact with the electrical groun.1, while some of the 6 identified wire crimps were observed ta be touching the sctuator housings. During the LOCA test, the wire crimp surface should be <

touching the electrical ground in order to detect any insulation resistance degradation during the LOCA condition.

2. During the 1987 and 1988 period, when Peach Bottom Units 2 and 3 were shutdown, the licensee identified numucus Raychem splices which did not meet the Raychen qualification crite- a (sealing length too L short, bending radius too small). Qualificat. ion of these splices was [

not established at the time they were identified.

3. During the 1987 and 1988 periot, the licensee identified in Unit 2  :

that Ideal wirenuts were used to connect Atkomatic solenoid valves

to the field wire in the Containment Atmosphere Dilution Analyzer racks. The qualification for these wire nuts was not established in .

that there was no EQ file or qualification data for these wire nuts  !

and they were not on the EQ Master List. t

4. During the 1987 and 1988 period, the licensee identified in Unit 2 that Amp butt splices were used in 8 solenoid valve circuits, 2 pressure switch circuits and 1 level transmitter. The qualification for these butt splices was not established in that there was no EQ ,

file or qualification data for these splices and they were not on the l EQ master list.

Appendix A 2 Pursuant to the provisions of 10 CFR 2.201, Philadelphia Electric Company is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the re:ults achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
