1.0 PURPOSE / DISCUSSION 1.1 To provide instructions for the operator following a Main Steam Line Isolation Valve (MSIV) failing closed, not including a complete MSIV isolation.

1.2 This procedure may interface with the reactor scram procedure ONEP 05-1-02-I-1, with several Emergency Procedures, and with the Emergency Plan. EP-1, Level Control, covers a complete MSIV isolation.

1.3 An MSIV may fail closed due to the loss of RPS power to both solenoids, loss of accumulator air pressure, or energizing of the tes t solenoid.

In RUN mode, a Reactor Scram may occur if, at high power the proper combination of MSIV's are > 6% closed. Also, if one MSIV goes full closed, the pressure spike could cause a flux spike on the Neutron Monitoring System suf ficient to cause a scram.

SYMPTOMS 2.1 Possible reactor scran due to a pressure induced high flux trip or Group I Isolation.

2.2 MSIV position indicating lights indicate valve closure.

n 2.3 Increase in Reactor power, reactor pressure, other main steam line flows, feedwater flow, and generator output transient.

2.4 Power spikes on the Neutron Monitoring Recorders 2.5 High flux annunciators on the Neutron Monitoring System 3.0 AUTOMATIC ACTIONS l1 3.1 Possible reactor scram 3.2 Possible Group I Isolation due to high flow in other steam lines. This may cause safety-relief valve actuation.

4.0 D4 MEDIATE OPERATOR ACTIONS 4.1 Verify automatic actions have occurred, as applicable.

4.2 If scram has occurred, refer to ONEP 05-1-02-I-1, Reactor Scram.

4.3 Determine and correct the cause of the closed MSIV.

Single MSIV Closure th) . : 05-1-02-I-3 Revision: 10 Page: 2 4.4 If scram has not occurred, reopen the closed MSIV per SOI 04-1-01-B21-1 5.0 SUBSEQUENT ACTIONS 5.1 Proceed to impleme nt the Emergency Plan if directed by the Shift Supervisor / Shift Superintendent. -

5.2 If the closed MSIV(s) have been reopened, continue to operate tne plant in accordance with I0I 03-1-01-2, Power Operation.

5.3 If a single closed MSIV cannot be reopened, reduce reactor power as directed by the Reactor Engineer and Shift Supervisor / Shift Superintendent.

5.4 Refer to Technical Specification Section 3/4.4.7, Main Steam Line Isolation Valves LCO.
