APPENDIX A NOTICE OF VIOLATION Pennsylvania Power and Light Company Docket No. 50-387 Susquehanna Steam Electric Station, Unit 1 License No. CPPR-101 Berwick, Pennsylvania As a result of the inspection conducted on March 22-26 and 29-April 2,1982 and in accordance with the NRC Enforcement Policy (10 CFR Part 2, Appendix C), 47 FR 9987 (March 9, 1982), the following violations were identified:

A. 10 CFR 50 Appendix B Criterion XVIII audits, states that a comprehensive system of planned and periodic audits shall be carried out to verify compliance with all aspects of the quality assurance program and to determine the effectiveness of the program, and Criterion II, Quality Assurance Program, states that the program shall provide control over activities ...

consistent with their importance to safety.

Contrary to the above on April 2, 1982 only 8 QA audits of the preoperational test progra.n had been conducted of only some aspects of the preoperational test program, for example,1) the aspect of the development of the ISG Startup Worklist from the construction turnover exception list for the importance to safety Q listed systems had no planned and periodic audits and 2) many Q-listed systems had no planned and periodic QA Audits, e.g. Q Listed Systems Nos. 4.0, 5.0, 6.0, 9.0, 11.0, 17.0, 45.0, 54.0, 55.0, 58.0, 60.0, 61.0, 62.0, 64.0, 69.0, 70.0, 71.0, 73.0, 75.0, 76.0 and 81.0.

This is a Severity Level IV Violation (Supplement I).

B. 10 CFR 50 Appendix B Criterion II states that " activities affecting quality shall be accomplished under suitably controlled conditions ... and assurance that all prerequisites for the given activity have been satisfied."

10 CFR 50 Appendix B, Criterion V, Instructions, Procedures and Drawings states that " activities affecting quality ... shall be accomplished in accordance with inctructions, procedures ..." PP&L's AD 6.4 Rev. 9 "Startup Work Authorization" states that "... All hardware identified under PP&L Quality Assurance Program will require an SWA ..."

Contrary to the above, on April 2, 1982 it was identified that the instal-lation of core spray pump suction line strainers, an activity affecting quality of preoperational testing, was accomplished without an SWA and the core spray pump preoperational test had been conducted.

This is a Severity Level IV Violation (Supplement I).

I 4 Appendix A 2 Pursuant to the provisions of 10 CFR 2.201, Pennsylvania Power & Light Company is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation. Where good cause is shown, consideration will be given to extending your response time.

The responses directed by this Notice are not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
