Therefore, the following revisions are hereby made:
1.
Article I - Statement of Work, is hereby modified as follows:
A.
Incorporate the following additional requirement under Paragraph 3.2, Subtask 2:
t "In accordance with licensee requirements, the contractor shall undergo an on-site radiation training program prior to conducting the survey."
B.
Refer to Paragraph 5.0 fleetings and Travel, and delete Item 2 in its g

Article II - Period of Performance is hereby modified by deleting that article in its entirety and substituting the following in lieu thereof:
" Article II - Period of Performance The performance of work described in Article I hereof shall commence on September 29, 1982 and all effort shall be completed and all deliverables received by February 28, 1983."
3.
Refer to Article III - Consideration and Payment, Paragraph A. Estimated Cost, Fixed Fee and Obligation, and delete Items 1 and 3 in their entirety and substitute the following in lieu thereof:
"1.
It is estimated that the total cost to the Government for full performance of this contract will be $21,736.94, of which the sum of $20,385.63 represents the estimated reimbursable costs, and of which $1,351.31 represents the fixed fee.
3.
The amount pre.ently obligated by the Government with respect to this contract is $21,736.94."
In summary, this modification:
1.
Incorpcrates the contractor's on-site radiation training prior to the licensee authorizing access to the site for the conduct of the site survey; 2.
Expands the site visit period by seven (7) days from four (4) to eleven (11) days; 3.
Extends the period of performance by an additiona two (2) months from December 28, 1982 to February 28,1983; and 4.
Increases the total estimated amount of the contract by $3,832.92 from
$17,904.02 to $21,736.94.
