- Pursuant to the requirements of Salem Generating Station Unit No. 1 Technical Specifications, Section 6.9.1.9.b and IE Bulletin 80-24, we.are submitting Licensee Event Report for Reportable Occurrence 81-64/0lT.

This. report was required within fourteen (14) days of the occurrence in accordance with IE Bulletin 80-24.

It is being submitted late because of oversight during a turnover of LER Coordinators.

Normally, equipment malfunctions are thirty (30) day reports, and this report was mistakenly filed as a thirty day report during the personnel change.

Containment Fan Coil Unit Service Water Leak..

This_ report'was initiated by Incident Report 81-255.

CONDITIONS PRIOR TO' OCCURRENCE:

Mode 1 - Rx' Power 96% - Unit Load 1000 MWe DESCRIPTION OF OCCURRENCE::

On July 15, 1981, during a routine" containment inspection, an operator discovered' service water leaking from a 3/4" vent connection pipe weld up stream of valve-15SW65 on No. 15 containment. fan-coil unit (CFCU),

at a-rate of approximately 1.5 gallons per hour.

In accordance with

'NRC IE Bu11etin.80-24, the NRC was notified'of the service water leak in contalument by telephone at 1300 hours0.015 days <br />0.361 hours <br />0.00215 weeks <br />4.9465e-4 months <br /> with written confirmation transmit ced within the next 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />.

No. 15 CFCU was removed from service'and the service-water leak isolated.

Action statement'3.6.'2.3.a was antered at 1430 hours0.0166 days <br />0.397 hours <br />0.00236 weeks <br />5.44115e-4 months <br />, July 15, 1981, after declaring the CFCU inop-erable due to service water isolation.

This occurrence constituted operation in a degraded mode in accord-ance with technical specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The service water leak was due to a. weld failure on a 3/4" pipe to flange' dissimilar metal weld.

ANALYSIS OF OCCURRENCE:

Technical specification 3.6.2.3.a requires:

With one group of containment cooling fans inoperable and both Containment Spray Systems operable, restore the inop-erable group of fans to operable status within 7 days or be in hot standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in cold shut-down within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

CORRECTIVE ACTION

The crackiwac'in a weld between a 3/4" carbon steel pa

'ind a stain-less steel. flange.

There was an existing design change request to change the dissimilar metals in that system to all stainless steel.

_The 3/4" carbon steel pipe was removed, and replaced with stainless steel. TNo. 15 fan coil unit was tested satisfactorily and returned to service.

At 2145 hours0.0248 days <br />0.596 hours <br />0.00355 weeks <br />8.161725e-4 months <br /> July 16, 1981, action statement 3.6.2.3.a was terminated.

The design change request was completed on Nos. 11, 12, 13 and 14

containment fan coil units.

No. 15 CFCU is partially completed and will be finished during the next suitable outage' period.
