Licensee Actions on Previcas Inspection Findings (Closed) Unresolved Item (483/81-13-01) Prior to performing work, Inryco did not have an established procedure to accomplish the removal and subsequent testing of a post tensioning wire from tendon V50.

It was verified that Inryco procedure F7.1-259 was developed and used to accomplish the task. The procedure was attached as part of the dispositioa to DIC NCR 2SN-4022-C.

Functional or Program Areas Inspected This inspection consisted of a review of Bechtel specifications and Inryco procedures as they applied to post tensioning activities underway at the Callaway site.

In process production and QC inspection activities relative to tendon installation and stressing were observed. Pertinent post tensioning quality records were also reviewed. Details of the inspection are documented in the following sections of this report.

A.

Observation of Work / Inspection Activities 1.

Installation The installation of over-the-dome tendons V22 and V18 on July 8 and 9, 1981, respectively, was observed. The installation of both tendons was in accordance with the Inryco Field Installation Manual, Chapter 5, Section 9.

Additionally, the inspection of tendon V18 by Inryco QC personnel was observed to be in accordance with the requirements of the Inryco Field Installation Manual, Chapter 9, Section 8 and Bechtel Specification 10466-C156.

During the installation of tendon V22, one wire became bent during the uncoiling procces. After installation was complete, the exposed bend was inspected by Inryco QC personnel to determine whether it exceeded the bent wire criteria in Chapter 5 of the Inryco Field Installation Manual. The results of the inspection demonstrated that the bend in question did not constitute a non-conforming condition. The RIII inspector coecurred with the Inryco inspector's conclusions regarding the acceptability of the wire.

Stressing The stressing of horizontal tendon 36BA was observed on July 9, 1981. The stressing operation was verified to be in accordance

with the requirements of Inryco Field Installation Manual, Chapter 7 and Bechtel Specification 10466-C156. Inryco QC personnel were observed conducting their inspection of the stressing work in accordance with the Inryco Field Instal-lation Manual, Chapter 9, Section 10.

production personnel were reviewed. The personnel met the requirements set forth in the Inryco Field Installation Manual, Chapter 9, Section 4.

The qualification and training records for one Level II and two Level I QC inspectors werc reviewed. The inspector's certification records met the requirements of ANSI N45.2.6 as referenced by the Inryco QA Manual,Section II.10.7.

Review of Installation, Stressing, and Greasing Quality Records The tendon pulling, stressing, and greasing cards for over-the-dome tendons VI, V3, V5, V7, V9, V11, V13, V15, V17, V19, V21, V23, V25, V27, V58, V60, V62, V64, V66, V68 and horizontal tendons 25CB, 27CB, 29CB, 31CB, 33CB, 26BA were reviewed. The review confirmed that the specified streasing sequence had been followed.

It also verified that the allowable time limits between instal-lation/ stressing and stressing / greasing, as specified by Bechtel Specification 10466-C156, had been met, with the following exceptions.

a.

DIC NCR 2SN-3902-C identified four tendons (V1, V43. V44, and V86) which were not greased within fif teen calendar days after stressing. The fifteen day greasing requirement is specified in Bechtel Specification 10466-C156.

b.

DIC NCR 2SN-4022-C identified two tendons (V7 and V50)

which were not greased within the specified fifteen days afte. stressing.

DIC NCR 2SN-3993-C identified six tendons (V19, V25, V58, V62, V68, and V72) which were installed and not strecsed within sixty days. The sixty day stressing requirement is specified in Bechtel Specification 10466-C156.

The disposition of each NCR was verified to be in accordance with Inryco procedures and was consistent with good construction practice.

Exit Interview The inspector met with staff representatives (denoted in Persons Contacted paragraph) during and at the conclusion of the inspection to summarize the scope and findings of the inspection.
