March 24, 1977 Occurrence Date March 14, 1977 identification of Occurrence Violation of the Technical Specifications, paragraph 3.6. A.3, when the radio gases released from the stack were not continuously monitored. This event is considered to be a reportable occurrence as defined in the Technical Specifications, paragraph 6.9.2.A.2.

Recirculation, 13 5 x 10 6 lb/hr Feedwater, 5.03 x 106 lb/hr Stack Gas: 11,400 pci/sec Description of Occurrence On Monday, March 14, 1977, at approximately 1143 hours0.0132 days <br />0.318 hours <br />0.00189 weeks <br />4.349115e-4 months <br />, a " Stack Gas Low Flow" alarm sounded in the station control room. Investigation revealed "A" that "B" stack gas stack gas sample pump had tripped. At approximately 1146, sample pump was put in service and "B" stack gas sample pump was isolated.

An electrician was dispatched and found no immediate cause for pump trip and -

at approximately 1201, "B" stack gas pump was placed back in service in order to attempt to repeat the trip or determine the cause of the previous trip.

On Tuesday, March 15, 1977, at approximately 0135 hours0.00156 days <br />0.0375 hours <br />2.232143e-4 weeks <br />5.13675e-5 months <br />, a " Stack Gas Low"B" Flow" alarmed in the station control room. Investigation revealed "A" that stack gas stack gas sample pump had tripped. At approximately 0139, sample pump was put in service. "B" stack gas sample pump Isolated and the

" Stack Gas Low Flow" alarm cleared. An electrician was dispatched and again fcund the pump tripped on thermal overload. The pump was removed and taken to the machine shop for dismantilng to determine the cause.

Apparent Cause of Occurrence The cause for the thermal overload trip was attributed to failure of the diaphram operated valve in the lubricant recirculation system for the pump.

\ /Md Reportable Occurrence No. 50-219/77-2-lT Page 2 j March 4, 1977 Analysis of Occurrence The safety significance of this event is considered to be minimal, since

.during the 3-minute period and the 4-minute period the sample system was inoperable, no increases were noted in off gas activity or in area radiation monitor readings. Furthermore, when returned to service, the stack gas monitors indicated the same as they did before the failure. It can, therefore, be concluded that the radio gases emitted from the stack were stable and constant during the period of inoperability.

Corrective Action immediate corrective action was to isolate "B" stack gas sample pump and put "A" stack gas sample pump into service. After the second trip, "B" stack gas sample pump was dismantled for inspection and replaced with a spare. An investigation will continue as to the feasibility of an automatic start feature for the standby pump.
