This event is considered to be a reportable occurrence as defined in the Techni-cal Specifications, paragraph 6.9.2.b.2.

Conditions Prior to Occurrence Startup testing following a refueling / maintenance outage was in progress. The major parameters at the time of occurrence were:

Power: Reactor, 288 MWt Generator, O MNe Flow: Feedwater, 1.2 x 106 lb/hr Recirculation, 5.3 x 10" GPM Stack Gas, .7 x 10" pCi/sec Description of Occurrence On July 16, 1980 during the routine functional testing inspection of hydraulic shock and sway arrestors (snubbers) installed on safety related systems, snubber 75/23 (serial number 469950) failed to lock up in the tension direction. The snubber was replaced with an operable spare and subsequently disassembled.

Inspection of the valve block internals revealed a nick on the poppet on the tension side which apparently caused excess bleeding of hydraulic fluid and prevented lock up.

Apparent Cause of Occurrence The cause of the occurrence is attributed to component failure.

Analysis of Occurrence l Snubbers are intended to limit piping movement during transient and seismic events. The functional testing of a representative sample (10% minimum) of all hydraulic snubbers is a surveillance item performed each refueling cycle per technical specification requirements (Paragraph 4.5.Q.4).

Investigation of surrounding attachments on this piping system (Isolation Con-

denser "B") reveals that due to the proximity of operable snubbers, hangers and guides in the vicinity of the inoperable snubber and the low probability occurrence of a seismic event, that the safety consideration of this incident i is considered to be minimal.

Corrective Action l!

I This snubber was replaced with an operable spare.

Selected hydraulic snubbers on isolation condenser "S" were tested to verify operability.

l After the defective poppet was replaced and new fluid was installed, the snubber was retested and performed satisfactorily.
