Exceeding a limiting condition for operation as per Technical Specifications, paragraph 3.1.A.2, (Table 3.1.1-Item A12) when pressure switch PSL "D" was i found defective and inoperable. l This event is considered to be a reportable occurrence as defined in the Techni-cal Specifications, paragraph 6.9.2.a.2.

Conditions, Prior to Occurrence The plant was shutdown for a refueling / maintenance outage.

The reactor was suberitical.

The reactor mode switch was locked in refuel.

The reactor cavity was flooded.

Description of Occurrence On April 20, 19SO, while performing surveillance on generator load rejection sunsors, pressure switch PSL "D" was found defective. Investigation revealed that the pressure switch would not have tripped at any pressure due to a nis-located operating lever withia the switch hcusing. On May 12, 1980 during a review of a preliminary report, it was recogniced that this event constituted m it..ediate report.

Su veillance procedure records indicate the PSL "D" switch performed satisfc.c-

rrily on 12/2C!79 when the last strveillance test was performed. Therefore, f.e switch is ccnsidered to have been inoperable betweer 12/20/79 and 1/5/SO the 19S0 Refueling Outage began.

j redundant sensors, PSL "A" and "C" in the independent trip channel.

Arnarent Cause of Occurrence The cause of this occurrence is attributed to component failure.

Analysia of Occurrence The generator load rejection sensors are pressure switches sensing hydraulic oil pressure on the turbine acceleration relay. There are four sensors; two redundant sensors in each of two independent trip channels.

', The function of these sensors is to provide an anticipatory scram signal when load is rejected from the generator to minimi:e reactor pressurization transi-ents. The safety significance of the failed sensor is considered minimal rince l the redundant trip channel sensor was cpecabic and the independent trip channel was operable. Therefore, the trip function was not lost and a scram would have resulted if a generator load rejection had occurred.

4 i Corrective Action

, The defective pr, essure switch was disassembled and inspected, it was found that a teflon actuator that activates the switch was cocked and caused the failure.

4 The actuators on the other svi:ches were checked te verify that this condition i did not exist.

Since the plant was shutdown for a refueling outage, the switch was removed and l will be replaced with a new switch prior to plant startup.
