Identification of Occurrence Stapdby Gas Treatment System #1 was found to be inoperable during a scheduled surveillance test. . ,

This event is considered to be a reportable occurrence as defined in the Techni-cal Specifications, paragraph 6.9.2.B.2.

Conditions Prior to Occurrence The plant was shutdown for a refueling / maintenance outage.

  • The reactor. mode switch was locked in_ refuel. l The reactor cavity was flooded and less than 212*F. l Description of Occurrence On Tuesday, February 19, 1980, at approximately 2130 hours0.0247 days <br />0.592 hours <br />0.00352 weeks <br />8.10465e-4 months <br />, during a scheduled surveillance of the Standby Gas Treatment System (SGTS), a trip was experienced on SGTS #1 and automatic circuitry started SGTS #2 which performed satisfactority.

A s'ubsequent investigation ~-revealed that the absence of a system flow signal resulted in a shutdown of SGTS #1.

Apparent Cause of Occurrence The cause of the occurrence was due to a cracked flow sensing line (tygon tubing) which provided an erroneous signal to the system control circuitry indicating no system flow. The sensing line is considered to have failed as a result of being

~ stepped on by station personnel.

.the plant-stack. Although_SGTS #1 was observed to be Inoperable when the system i was initiated, the control; logic did, as designed, initiate SGTS #2 which proved

l to be functional.- The safety significance of this event is considered minimal  ;

since the only concern was a loss of redundancy in the SGTS's. ,

s Corrective Action SGTS #1 was removed from service and the defective flow sensing line was replaced.

An operability test was performed.to prove the system operable. The plant engi-

-'neering staff will evaluate the' current design of the SGTS flow-sensing line to

. prevent a recurrence of this type.
