This event is considered to be a reportable occurrence as defined in the Technical Specifications, paragraph 6.9.2.a.6.

Conditions Prior to Occurrence The plant was operating at steady state power. The major parameters at the time of occurrence were:

Power: Reactor, 1914 MWt Generator, 664 MWe Flow: Recirculation, 15.4 x 104 gpm Feedwater, 7.16 x 106 lb/hr Stack Gas: 3 2 x 104 pCl/sec.

Description of Occurrence On Monday, October 8,1973, at approximately 1630 hours0.0189 days <br />0.453 hours <br />0.0027 weeks <br />6.20215e-4 months <br />, an operator discovered both personnel access air lock doors in the northeast corner of the Reactor Building (door #19) were open simultaneously. Since this was a breach of secondary containment integrity, both doors were immediately closed. The operator then reported the occurrence to the Group Shift Supervisor. The apparent cause of this condition was attributed to contractors using this passage to bring materials into the Reactor Building. A security round was made around 1430 hours0.0166 days <br />0.397 hours <br />0.00236 weeks <br />5.44115e-4 months <br /> and verified both doors were closed.

Apparent Cause of Occurrence Contractor personnel failed to recognize a violation of the Technical Specifi-cations. ,

Reportable Occurrence No. 50-219/79-34/1T-0 Page 2 October 22, 1979 Analysis of Occurrence Secondary containment is required to minimize ground level release of airborne radioactivity materials, and to provide for controlled, elevated release of the building atmosphere under accident conditions. The ability of secondary containment to perform its function with both air lock doors open was degraded.

Considering the length of time concerned, the safety significance is minimal.

Corrective Act iqn Since the automatic closing device on one of the doors was disconnected, it was reconnected. The mechanical interlock was checked and was working properly. A Group Shift Supervisor spoke with the contractors on site to explain some of the operating requirements of the plant and all contractor foremen and Job supervisors were informed of the circumstances. The importance of not defeating mechanical locking mechanisms on site doors will be in-corporated into the plant's indoctrination course. Also, signs with the appropriate precautions will be placed on applicable doors.
